Dental Plan: Change • Current carrier is Ameritas Insurance. • New carrier effective 6/1/12: Anthem BC/BS • Offered Lower Premium Increases • Discount on Health Insurance Premium • No waiting period on services after first offering. • Rollover of unused benefit up to $250/year to a maximum of $1,000.

Annual Maximum Limit $2,500 for this year. Current plan year can incur claims through August 31, 2012. Have 2 months to file claims until October 31, 2012.

How do the Health Reimbursement Accounts (HRA) plans work? • Anthem provides KC 30/2,000 Plan • This plan has: • $2,000 single /$4,000 family deductable • 80/20 co-insurance • $30 co-pay for Primary Care, $50 co-pay for Specialists • Max. Out of Pocket $4,500 single / $9,000 family • Higher deductable means lower premiums

How do the HRA’s work? • Shickel Corporation sets up a Health Reimbursement Account (HRA) with LD&B for each plan. • Shickel has LD&B administer the HRA plan as either a: • $500 single / $1,000 family deductible plan or • $1,000 single / $2,000 family deductible plan • With the savings from the premiums, Shickel then pays 80% of all claims over the deductible up to the $2,000. • After the $2,000 is reached then Anthem starts to cover up to the maximum out of pocket limit. • Shickel is self insuring, taking the risk for the gap between the deductibles and $2,000 at 80%.

  25. How do the HRA’s work? • These plans require participants to take all Explanations of Benefits (EOB’S) they get from Anthem and file them with LD&B. • All claims received by midnight Thursday, get paid to you by the next Monday by check or direct deposit. • You are then responsible for paying the service provider directly.

Anthem Changes Changes for all plans: • Home health Care limit going to 100 visits per member per year from 90 visits. • Maternity benefits now cover dependents. • Expanding foot care coverage of corns, calluses and care of toenails as treatment only for patients with diabetes or vascular disease. • Excluding coverage for sexual dysfunction drugs • Excluding coverage for removal of wisdom teeth • Autism Spectrum Disorder Mandated added.

Anthem Changes Changes for the HRA plans only: • In-network Specialist co-pays increasing to $50 from $30 • Spinal manipulation - $25.00 copayment for services, currently 20% coinsurnace • Hospice Care – removing member cost share. • Mental Health/Substance abuse outpatient office visits require a PCP copayment.
