Enrollment • Autoenrollment for medical insurance only (at individual level) • All other coverage must be elected through employee self-service • Changes are only permitted during specified enrollment periods: • 30 days within date of hire • Annual Enrollment (October/November timeframe for January 1 effective date, will be announced via email and on Global) • Qualified Life Events (birth, death, marriage, loss of other coverage, gain of other coverage, etc) – If you have questions about your situation, contact the benefits unit to check eligibility: benefits@crs.org • Eligible dependents include: • Legally married spouse (does not include same-sex spouses) • Dependent children (up until age 26) • Documentation is required to approve dependents and finalize enrollment

  5. Aetna International Medical Insurance

  6. Medical Insurance - Overview Administered by Aetna International Works worldwide (same as plan for international employees) One plan with automatic enrollment at the individual level Vision coverage is available and bundled in the plan (uses the EyeMed network) The CRS plan is designed to adhere to Catholic Social Teachings. For information on covered and excluded services, refer to the plan document. Most services are covered at 90% with no deductible Wellness/preventative services covered at 100% Two-tier prescription plan ($10/$20), with mail order option available

Medical Insurance – In-Network vs Out-of-Network • Coverage is provided both in-network with Aetna or with out-of-network providers. • Use in-network providers for the highest level of reimbursement • A deductible will also apply to out-of-network services • Charges over the Aetna allowed amount with out-of-network providers are not reimbursable by the plan, nor will they apply to your out-of-pocket limits for the year.

Medical Insurance – Teladoc (U.S. only) • Teladoc is a national network of U.S. board-certified doctors available on-demand 24/7/365 to diagnose, treat and prescribe medication, if necessary, for many of your medical issues • When can Teladoc be used? • When the physician is unavailable: no appointments; after hours  • Schedule doesn’t permit traveling to see your physician (work, etc.)  • On vacation or a business trip  • For refill of recurring prescription (short term only)  • Pediatric care for any age • Included in your medical plan, visit www.Teladoc.com to enroll

  9. Delta Dental Domestic Dental Insurance

Dental Insurance • Administered by Delta Dental • One plan available, requires enrollment through self service to obtain benefits • Semiannual preventative care is covered at 100% with no deductible • Restorative care available at 50% or 85% coverage (depending on service) with $50 deductible • Orthodontia coverage available regardless of age at 50%, with a $2,000 lifetime maximum • Separate ID card from medical coverage

Flexible Spending Accounts (FSA) • Pretax savings vehicles for medical and dependent care • 2018 limits: • Medical - $2,650/year • Dependent Care - $5,000/year • If you have contributed through another employer, your annual limit is across all FSA plans, so you will have to take that into account when enrolling • Requires annual enrollment (does not automatically renew) • Claims must be incurred by March 15th of the following year, with all paperwork for claims submitted no later than April 15th • Any unused money at that point will be forfeited, so make sure that you are only saving as much as you need and submitting claims on time

  15. International SOS Emergency Medical Evacuation

Emergency Medical Evacuation • Administered by International SOS • 100% paid by CRS • Provides a medical emergency evacuation service for domestic and international employees when traveling/living overseas for CRS • When there is an immediate need for medical care and this is not available in-country, transportation is provided to the nearest appropriate medical facility • Does not cover the cost of medical care, only the transportation • Claims are opened directly with International SOS for approval

  17. AXA Short Term Disability

Short Term Disability • Administered by AXA • 100% paid by CRS, automatic enrollment after 6 months of employment • Provides 60% of pay when unable to work due to disability, up to a maximum of 6 months • Starts as soon as the second week of absence, but all paid time must be exhausted prior to starting benefits • Application is filed with AXA, but must also be communicated to Benefits with the filing of a request form • Will run concurrent with any Family Medical Leave (FML) time

Long Term Disability • Administered by AXA • 100% paid by CRS, automatic enrollment after 6 months of employment • Provides 60% of pay when unable to work due to disability for more than 6 months, can continue to age 65 or until the end of the disability • Application and paperwork are filed with AXA following the exhaustion of the Short Term Disability 6 month period

Maternity Leave • Administered by CRS Benefits – Leave Request Form signed by manager and submitted to CRS Benefits for approval • 100% paid by CRS, automatic enrollment after 6 months of employment • 6 weeks of paid leave following a normal birth, 8 weeks of paid leave following a Cesarean section • Begins payment on the first day following date of birth • 12 weeks of job-protected leave available under the Family Medical Leave Act (FMLA), up to 16 weeks of leave available with supervisor approval • Vacation and Personal Business leave may be used after initial 6/8 weeks, unpaid leave available after paid leave is exhausted

Parental Leave • Administered by CRS Benefits – Leave Request Form signed by manager and submitted to CRS Benefits for approval • 100% paid by CRS, automatic enrollment after 6 months of employment • 2 weeks of paid leave for regular domestic and international employees who are new fathers, foster parents and adoptive parents to allow time for you to bond with your new child • Must be used in the first 3 months after the arrival of the child • 12 weeks of job-protected leave available under the Family Medical Leave Act (FMLA) • Parental leave days may be used intermittently or concurrently • Vacation/personal leave must be exhausted before unpaid leave is taken

Adoption Leave • Administered by CRS Benefits – Leave Request Form signed by manager and submitted to CRS Benefits for approval • 100% paid by CRS, automatic enrollment after 6 months of employment • 6 weeks of paid leave for regular domestic and international employees who are new adoptive parents to allow you time to bond with your new child (not available for step children) • Must be used in the first 3 months after the arrival of the child • 12 weeks of job-protected leave available under the Family Medical Leave Act (FMLA) • Parental leave days may be used intermittently or concurrently • Vacation/personal leave must be exhausted before unpaid leave is taken

  26. Basic Life/AD&D Insurance • 100% paid by CRS, automatic enrollment on date of hire • Life insurance and AD&D covered at 2x annual salary for each (maximum $500,00)) • No underwriting required

Supplemental Life Insurance • Administered by AXA • Up to 5x salary (maximum $500,000) available for yourself • Elected in 1x, 2x, 3x, 4x, or 5x salary increments • Amounts over $100,000 subject to approval • Up to $100,000 available for your spouse • Elected in $10,000 increments • Must not exceed 50% of your own supplemental coverage • Amounts over $30,000 subject to approval • Up to $25,000 available for your dependent children • Elected in $5,000 increments • Must elect supplemental coverage on yourself • No underwriting required

  29. 403(b) Retirement Plan • Administered by Transamerica • Automatic enrollment at 3%, can be changed at any time through Transamerica • After 6 months of service, CRS will contribute 7% of your annual base salary and will match 0.5% for every 1% you contribute, up to 6% of your base pay. • Enhanced match of 3% for employees earning under $45,000 per year • All contributions 100% vested • Pretax and Roth options available • Refer to retirement plan documents for more detail

  31. Employee Assistance Plan • Business Health Services • Available for all CRS employees (domestic and international) as well as anyone living in their household • Counselors are available to help with a range of work-life balance issues

  33. Other Benefits • Business Travel Accident (100% funded by CRS, automatic enrollment) • Available for regular domestic and international employees while they are traveling on CRS business. The death benefit is $500,000. This coverage and the travel benefits described below are provided through AIG, and this insurance is in addition to your CRS basic life insurance coverage. • Credit Union Membership (SECU) • Available to all CRS employees, enroll directly with SECU and identify yourself as a CRS employee • On-Site Fitness Center • Cardio/strength equipment available, as well as group fitness classes including yoga, kickboxing, boot camp, and more • $20/month done via payroll deduction, no commitment period • Lockers and showers available

Other Benefits (continued) • Educational Assistance • Up to $5,200 per fiscal year available for approved courses • Courses must appropriate to the employee's job or an integral part of their career development plans • Language Assistance • Up to $1,200 per year to support language training related to professional development and/or organizational needs. • Courses must be deemed appropriate to the employee's job or an integral part of their career development plans. • Adoption Assistance • Available following six months of service. • Eligible adoption assistance-related fees may be reimbursed up to $2,500 per adoption.

  36. Enrollment • Log onto CRS Global, under My CRS, click Employee Self Service • You have 30 days from your date of hire to enroll

  37. Enrollment Select the Benefits Tab, and then select "Health & Welfare" from the drop-down menu. You will be directed to a landing page that will guide you into the enrollment system.

  38. Enrollment – Tips and Reminders • Remember to get all of your dependent documentation into the benefits department ASAP. Your enrollment cannot be finalized until this is received and processed! • Ensure when enrolling that you have completed all of the submission steps. You should receive a confirmation email when your enrollment has been received. • After enrolling in spending accounts (FSA or parking), you can access these accounts via single sign-on in employee self-service by clicking Benefits and then Spending Accounts. • Supplemental Life elections over the guaranteed issue must be approved by AXA. Please contact HR Benefits for the appropriate form.
