Wellness Program • Cary Academy is partnering with BCBSNC’s Healthy Outcomes Program • Available to all employees currently enrolled under the BCBSNC medical plan • This is a voluntary health promotion program that advocates accountability for healthy lifestyle behaviors • BCBSNC requires Employers to participate in one of their 9 Healthy Outcomes programs Program runs now through December 31, 2103!

Employee Portal • Top three program recommendations prominently displayed • Personalized messaging • Dashboard incentive status or activity tracking available • Education and media • Weight, nutrition, exercise, pain • Multiple health programs, tools and trackers • Calculators • Planners and guides

Employee Portal Features • Healthy Assessments – Helps identify health risk factors, readiness to change and other issues as well as helping measure productivity impact • Healthy Living Programs – 14 personalized, actionable, interactive online tutorials that promote positive health behavior changes • Healthy Conversations – 14 video and audio modules offering information and advice on several lifestyle and chronic condition types • Monthly Seminars – 10-15 minute monthly seminars on various health and wellness topics • Newsletters – Easy-to-read monthly newsletters featuring tips, articles and recipes for a healthy lifestyle

  6. Preventive CareAge/Gender Appropriate Screenings The member must meet the following criteria as they apply, based on their age and gender:

What’s in it for us? • With 50% participation in the program we starting earning premium credits towards next year’s renewal cost The higher the level of participation, the greater the likelihood of receiving premium credits at a higher rate. Every dollar saved for the plan is money saved for you via payroll deductions. • Individually if you meet the 125 credits under the program you will receive a $25 gift card of your choice through the Charger Bucks Program • If we reach our targetgoal of 80% participation, everyone that reaches 125 will be entered into a raffle for a grand prize! * Such consideration does not guarantee an effect on rates.
