  10. COVER STORY Misti Foust-Cofield Vice President and Chief Nursing Officer Reid Health

  11. The following is an interview between Foust-Cofield and Insights Care, which discusses leadership, the healthcare sector, and her background in the field. We are accredited by the Healthcare Facilities Accreditation Program (HFAP), the Indiana State Department of Health, Medicare/Medicaid, American Association of Blood Banks and the American College of Surgeons Community Cancer Program. Misti, tell us about yourself and your professional journey so far. What are the key products and services of the organization? I had the privilege of starting my own clinical journey as a SICU/Neuro ICU nurse at Halifax Medical Centre in Daytona Beach, Fla. We served as a Level 2 Trauma Centre for Daytona and many surrounding communities, caring for post-trauma patients and their families in the area. In addition to our inpatient hospital in Richmond, Ind., Reid Health offers primary care and several specialty services at locations throughout our eight-county service area. About 14 years ago, I transitioned “home” to Reid. I came back to Reid as a bedside Open Heart Recovery Nurse and was blessed to work with many great physicians and colleagues. I moved through a variety of growth opportunities including Manager of CCU, Inpatient Director of Nursing, and eventually into the role of CNO. Key service lines include Heart & Vascular, where our providers have experience with the Watchman and TAVR procedures and cutting-edge treatment options to address conditions such as A Fib (Watchman) and aortic stenosis (TAVR). We also have an excellent cardiothoracic program to keep our community receiving open heart surgery close to home. Please tell us about Reid Health and its inception story. Other services include MSK, where we have made a particular commitment and investment in technology, including Mako and Mazor robotic systems for joint replacement and minimally invasive spine surgeries; Obstetrics and Gynecology services; Emergency Services; Surgical Services and Oncology. Reid Health offers nationally recognized quality and technology with a personal touch that sets us apart. Our growing team is known for going the extra mile to make sure our patients and families are made to feel as comfortable and cherished as possible when they find themselves in need of our services — on the main campus or at one of numerous satellite facilities in the eight-county region of Indiana and Ohio that we serve. We opened as Reid Memorial Hospital in 1905. Daniel G. Reid, an industrialist originally from Richmond, financed the construction. The hospital name was in memory of Reid's wife, Ella, and their son, Frank, who both died before the turn of the century. In 1992, we changed our name to Reid Hospital and Health Care Services to reflect our growth into much more than an inpatient hospital. In 2015, we again changed our name, this time to Reid Health. We’ve been rounding to simply acknowledge the hard work of our teams. They’ve been so commi?ed to our community, and we are indebted for the service and amazing care they con?nue to render. After almost 100 years at our original location, it became obvious the most cost-efficient and quality-driven option for continued growth would be a new facility. In September 2004, we broke ground at our new location on about 100 acres two miles north of our old home. We completed the move in September 2008, when the inpatient hospital opened its doors. Today, our health system includes a 271- bed hospital on our main campus and numerous satellite locations in the region of Eastern Indiana and Western Ohio that we serve. Reid's service area is home to about 280,000 people and includes six Indiana and two Ohio counties.

  12. Also, in 2019, we contracted with Wayne County, Ind., to begin offering ambulance services in one section of the county. Since then, we have grown to offer those same services to nearly all of Wayne County, as well as all of Union County in Indiana, and now have sevenambulances in our fleet. life balance by offering many of our meetings and communication through social media platforms and virtual options. This allows our teams to spend more time with their families. We’ve placed tremendous focus on the mental health and well-being of our bedside teams, offering EAP services and a Code Lavender support team. We’ve also undertaken a remodel of all staff breakrooms to develop tranquil, serene environments that our teams can use to take a mid-shift break that’s not in a sterile “hospital-like” environment. We recently opened a childcare center to help ease the burden of finding childcare. This has been wildly successful, so much so we have a wait list for openings at the center. What are the core values of Reid Health? How are you driving the organization to achieve its mission and vision? In April 2021, we announced new mission, vision, and value statements to help govern decision-making over the coming years. Mission: To lead our communities to well-being, one person at a time We’ve been rounding to simply acknowledge the hard work of our teams. They’ve been so committed to our community, and we are indebted for the service and amazing care they continue to render. Vision: Healthier people, thriving communities, trusted partner Could you share with us any moment or event that impacted your professional life profoundly? Values: Excellence, Empathy, Integrity, Accountability Those statements sum up our culture and give our team members a clear understanding of what’s important to us as an organization as we work to serve the needs of our patients and communities. For our patients and their families, the statements represent the level of care they should expect to receive from us. I’ve been blessed with many meaningful moments throughout my nursing journey. I’ve cared for a number of patients and families who have left a lasting impression on my heart and mind. One patient who I resuscitated during a cardiac arrest still stays in touch, and her children and grandchildren know me. They typically reach out on the anniversary of our encounter and send me words of kindness and appreciation. It’s that type of appreciation that fills my bucket, and I’m the one being truly blessed. As an experienced leader, what advice would you like to give to aspiring entrepreneurs and enthusiasts who wish to venture into the competitive industry that you are serving? In your opinion, what are the current challenges in the nursing industry? How do you aspire to turn them into opportunities under your leadership? We’re doing our best to reimagine nursing and nursing leadership. We’re partnering to increase our workforce pipeline and are focused on restoring the passion in our workforce and team members. We’re focusing greatly on the mental health and well-being of our clinicians and doing our best to enhance care delivery by leveraging technology and decreasing the stress on our bedside teams. I would highly recommend anyone considering this career to do it! Do it with all your heart, mind and soul. I would encourage others to be fearless and relentless to pursue your passion to serve others. This work is tough and can be stressful for not only the leader but their family. Amongst the heavy workflow, how do you ensure that your organization allows its staff to maintain a healthy work-life balance? Nursing leadership is more than a career, it’s a lifestyle and truly never turns off. If it’s who you are to the core, this is the appropriate field for you. Do not be afraid to fail and always be vulnerable and genuine to those who you’re privileged to serve. 14. The project will allow us to use our recently purchased patient beds and a new call light system to their fullest capabilities, which will enhance patient care and consolidate work for bedside staff. Give us a few of your awards or recognition that accurately highlight your company’s position in the market. During my time as a CNO, my team worked together to achieve several accreditations, certifications, and other recognitions from national organizations that reflect our high- quality care, including: Ÿ Verification as a Level 3 trauma center for our emergency rooms in Richmond and Connersville, Ind. Ÿ Certification as a Primary Stroke Center from the Healthcare Facilities Accreditation Program (HFAP) Ÿ Accreditation for the Reid Health Breast Center Ÿ Accreditation for our Urgent Care locations in Richmond, Connersville and Eaton, Ohio Ÿ American Heart Association’s Get with the Guidelines—Stroke Gold Plus award for best practices in stroke care Ÿ Accreditation for the Reid Health Care Pavilion Ÿ Accreditation for our Cardiac and Pulmonary Rehabilitation Programs in Richmond and Connersville others and to touch so many patients and families through the ripple impact of leadership. What are the future goals of the organization? How does it envision scaling up its operations to reach 2022 and beyond? In addition, Reid Health was named one of America’s Best Midsize Employers by Forbes and received the 2022 Outstanding Patient Experience Award from Healthgrades. We’re in the middle of an expansion that includes new facilities in key communities aimed at bringing more services and specialty care providers closer to the patients we serve. As part of that growth, we’re prioritizing technology, seeking to make state-of-the-art equipment and systems available to those who entrust us with their care. That includes a first-of-its-kind project that began this year to upgrade inpatient rooms at our hospital to add smart boards and other features that are directly integrated with Epic, our electronic medical records-keeping system.

What are some of the challenges you face when conducting nursing operations, and how do you turn them into opportunities for growth? I have never really sought a promotion in my nursing career. However, I have enjoyed my roles. The more nurses that I manage, the more I feel responsible for supporting them to reach their potential, strive towards the highest quality care and enjoy what they do. We have a global challenge regarding nurse recruitment and retention. This is a widely held concern for healthcare providers all over the world. To meet the needs of our future patients, we need to remodel how nursing care is delivered. For example, instead of calculating the number of nurses to patients in a ratio setting, we need to turn this upside down and consider what every individual needs and who is best placed to provide this care. This cannot only mean registered and licensed nurses – there are not enough of us – but this could include other trained and competent healthcare workers. What role do compassion and nurse-patient communication play in providing quality care for patients? When I speak to young nurses today, I am often called to give them simple tips on what it takes to be a good nurse. Of course, you need to be well-trained and competent in what you do. Still, with kindness and common sense added in, you are bound to make a massive difference in the quality of care of your patients. Please share with us some nursing insights you would like other nurses to know. Considering that you are an enthusiast in the nursing field, what nursing initiatives are you actively involved in? Being a nurse is a privilege. We are honoured to be invited into our patients' most personal and private world and allowed to offer what we can to make life easier, comforted and less painful. Develop a learning culture; Use nurse-process reviews to learn from serious incidents and near misses; Introduce Every Ready Governance, Comprehensive Unit-based Safety Programme, Pathway to Excellence and Work towards Magnet. How do you envision scaling your services and operations in 2023 and beyond? Every patient has the right to expect safe, caring and high-quality care. We are here to meet the needs of our patients and will stop at nothing to be able to deliver this. 19| January 2023|www.insightscare.com

  20. The Role of Chief Nursing Officers in Advancing the Healthcare Industry A quality, patient-centered healthcare system. From the highest person, the Chief Nursing officer, to the staff nurse, nurses must understand how nursing practice must drastically change to provide the expected level of quality care to achieve these outcomes and actively participate in the change. ccording to Advancing Health, nursing has a significant role in healthcare reform and the requirements for an affordable, secure, high- With a renewed emphasis on patient-centered care, care coordination, data analytics and quality improvement, these modifications will necessitate a new or enhanced skill set in wellness and population care. At the individual or system level, transformation and the necessary changes will not be simple. Individually, it necessitates assessing one's knowledge, abilities and attitudes to determine whether or not they position you to contribute to the upcoming change or to resist it. It necessitates an examination of the organization's mission, goals, partnerships, procedures, leadership, and other essential components at the organizational level before 20| January 2023|www.insightscare.com

  22. reorganizing them, thereby causing disruptions to the status quo. time through a seamless array of health, mental health and social services at all levels and intensities of care. Continuous coordination is essential to prevent or minimize fragmentation because the continuum includes services from health to illness, from birth to death, and from various organizations, providers and services. Let's go over the developments and learn more about them! Wellness and Prevention We must move away from a healthcare system that focuses on illness and toward wellness and prevention. As a result, wellness and preventive-focused evaluations, fitness and health education programs, and programs to address the social or environmental causes of preventable disease conditions and care issues must be given the same importance as providers' disease-focused clinical intervention. What does this entail in the context of actual orthopedic practice? It means addressing "upstream" factors to prevent or minimize musculoskeletal health issues at the population health level. All patients require care coordination because it bridges the disjointed health system and makes it more manageable and coherent, which benefits both the patient and the provider. Because it has been confirmed that transitions represent high-risk periods for safety issues and adverse outcomes due to a lack of continuity of care, this type of care, referred to as transitional care, has been a significant focus. Focus on Getting Better Results and Outcomes Nurses in the new healthcare system will need to be able to collect and track clinical and financial data across settings and over time. Key metrics need to be followed at the individual and population levels. It provides the necessary information to comprehend the challenges facing each patient. From a wellness perspective, a gait analysis, screening for osteoporosis, and a plan to prevent or reduce risk to prevent subsequent falls and potential fractures would all be implemented. The patient might also be referred to a Matter of Balance program, which would provide the patient with strategies to reduce falling and increase strength and balance. To achieve the triple aim, we will need to keep track of overtime of outcomes related to psychosocial status, behavior change, clinical and health status, satisfaction, quality of life, productivity, and cost. Predictive modeling employs these data to stratify the population based on the disease state or risk profile. Through stratification, health promotion, wellness and care will be provided to those at no or low risk. Patient and Family Focused Care One more important attribute of the changed medical care framework should be a faithful spotlight on the patient. If patients and families are to take charge of their health, they must receive care focused on them and their families rather than the providers. In conclusion, Again, nurses are well-suited for this position because they have always embraced a holistic approach to care that includes patients, families and communities and empowers patients to take responsibility for their health and disease management. Undeniably, nurses are well-positioned to assume responsibilities that will advance health, enhance care, and increase value. However, it will necessitate new approaches to thought and practice. Nurses can work with patients to help them achieve their goals by providing education and coaching with an eye toward the outcomes reported by patients. A necessary skill is using action planning and motivational interviewing to help patients change their behavior. “The time has come for nursing, at the heart of patient care, to take the lead in the revolution to make healthcare more patient-centered and quality-driven," reads the statement. "Nurses can no longer take a back seat." The vital question to ask here is, "Are you ready?" Sitting as the Chief Nursing Officer of Al-Futtaim Health, she is one of the most influential healthcare executives to know. empathetic, patient centric care continues to motivate me to this day. Can you shed some light on your career journey over the years? Originally from the UK, I trained and worked in the NHS for 15 years before relocating to the Middle East. I have passed through many roles in my career from staff nurse, educator, clinical nurse specialist, senior advisor, and nurse consultant. Each of these roles has positively contributed to where I am today. I was CNO of several facilities and groups in the region before being appointed as Chief Nursing Officer of Al Futtaim Health, leading the nursing functions across all brands and divisions of Al Futtaim. Al Futtaim has made its mark as a key player within the healthcare industry. Led by a dynamic team of industry experts, this year saw the managing director, Dr Haidar Al Yousef winning CEO of the year, and Chief Nursing Officer Siân Collier-Jones receiving the outstanding leadership award demonstrating the force behind the name. After securing the remarkable achievement of being the first primary healthcare chain in the UAE to be awarded international accreditation, Al-Futtaim Health went on to partner with the American Heart Association (AHA) and launch an international training center, dedicated to clinical education & life support training. My nursing administration & leadership skills organically evolved over time and were formally assessed when I was conferred as a certified global nurse consultant (CGNC) by the International Council of Nurses (ICN). 28. Can you elaborate on the core values and mission of Al- Futtaim Health? How do you align the nursing services to best serve the organization? Compassion is massively associated with nursing and is at the heart of what we do. I am incredibly grateful to have introduced the international DAISY awards to the organization. DAISY is a patient nominated award to recognize extraordinary nursing care and compassion. This year we were honored to welcome the DAISY foundation Co-founders, CEO and VP from the USA, to attend one of our nursing award ceremonies. Al-Futtaim Health’s values are centered around 5 key pillars: Excellence, Compassion, Innovation, Respect and Ethical practices. Being responsible for the creation and development of the nursing structure, excellence was fundamental. I believe excellence starts with a strong team culture, so nurturing & developing the right culture within the nursing department was critical to me. I ensured sustainable quality governance structures were embedded within the department, through dedicated roles, support & education. Innovation is the value that also relates to our mission, which is ‘to redefine the healthcare experience by delivering high quality, smart and integrated care’. This is one area where Al-Futtaim Health have been a pioneer in the region by creating end-to-end digital solutions, and nurses have been a key player in the care delivery of these solutions. Nurses’ roles are ever evolving, and the use of technology, telemetry and assisted devises are now part of modern-day nursing care. For example, we recently launched our SmartDoc project, this technology uses a remote device deployed & operated by specialist nurses. This project was a huge success for the organization and was even awarded platinum status at the 2022 Patient Safety Awards. In what areas, in your opinion, does the regional health system struggle when it comes to treating patients? In any private healthcare system, fragmented care is a real issue. It's essential to have a well-balanced healthcare ecosystem that works seamlessly for continuous health management. We live in ever-changing times, and the UAE has demonstrated its enormous strength and forward- thinking in this area. The pandemic saw instant responses and initiatives from the entire healthcare spectrum, both public and private. Al Futtaim Health was one of the first private providers to partner with the DHA on pandemic response initiatives, working together for population safety in real-time. We rapidly established a large COVID-19 field hospital and launched multiple response projects across Dubai. Considering that you are an enthusiast in the field of nursing, what nursing initiatives are you actively involved in? I am a huge advocate of nurse empowerment and I believe it’s important to constantly learn, share and contribute to developing the nursing profession. I have a passion for mentoring as it offers bidirectional learning and development as well as a connection to the

As a fellow of a government- selected nurse leader task force and roundtable expert panel, I can contribute to think tank policy development and promote strategic initiatives within healthcare. I was extremely humbled to have received an award earlier this year for my contribution to healthcare. Share with us some nursing insights or tips that you would like nurses to know. Never stop learning or growing. Nursing is such a versatile career with many opportunities. Every role in healthcare is important. Having worked across a spectrum of designations from HCA to CNO, I can't emphasize enough the importance of a systems mindset. Each role adds a unique value, and we all need to work together for care to be effective. Listening is how I learn so much from my team; it's also part of our motto, "We Hear You." It is important that you stay connected, as you move from the bedside to the boardroom, it is easy to get consumed by administration and lose connection with the wider workforce. Maintaining communication and ensuring psychological safety within your team is vital. In addition to structured feedback, leadership rounds, nursing committees and initiatives, I hold agenda free drop-in sessions monthly, which helps keep dialogue open. What are some of the challenges you face when conducting nursing operations, and how do you turn them into opportunities for growth? I think the biggest challenge faced by any CNO is staff retention or staffing numbers in general, as this impacts skill mix planning and general operations. This is a global issue everywhere and was recently intensified due to the pandemic. The surge in operational demands made everyone re-evaluate capacity planning. One thing that nurses are renowned for is their ingenuity and flexibility, so of course this has been turned into an opportunity for growth! re-evaluated, and we introduced modified processes, designations, or smart technology for greater utilization and skill optimization. This in turn created new roles and expanded opportunities for our nurses, with career and skill progression that ultimately resulted in staff satisfaction. In an ever-competitive global market, staff satisfaction and self-development are vital components of growing any workforce. We learned fast that with the right support, training and framework in place, it was possible to create a more agile, cross-functional workforce. Let's look at some of the transformative changes Nurse Shortage would Continue Around the world, there are 28 million nurses, according to the World Health Organization (WHO). However, that number isn't sufficiently high for patient numbers and needs. As a result, there is a global shortage of nurses, and to meet the healthcare needs of all patients and their needs, 6 million more positions will need to be filled by 2030. This number will online keep on ascending all through 2023 also. Many of the Gen X-er ages are close to retirement age, Coronavirus hindered new likely medical caretakers from acquiring training, and as medical caretaker burnout increments, more medical attendants are leaving the medical care field overall. The consequences of Coronavirus and the worldwide nursing deficiency are felt all through medical services and will continue being judged for the following couple of years. In its annual National Healthcare Quality and Disparities Report, the AHRQ has shown that when accessing healthcare, racial and ethnic minorities and low-income individuals face more obstacles and receive lower-quality care. These facts emphasize the need for our system to be changed. Covering with the changing social socioeconomics is the adjustment of illness-type socioeconomics since there is an ascent in persistent sickness among Americans and fundamentally so among more established Americans. An estimated 133 million Americans are affected by chronic disease, which includes heart disease, stroke, cancer, Type 2 diabetes, obesity, and arthritis. The leading cause of death and disability is chronic disease. Chronic diseases are among the most "common, expensive, and preventable" health issues. Driving Force for Change Healthcare Reform With the changing driving force, nursing has brought many changes in the medical field. The Patient Protection and Affordable Care Act of 2010 was ushered in by the continued skyrocketing of healthcare costs, the less-than-impressive health status of the American people, safety and quality issues within the healthcare system, and growing concerns that price and quality issues would intensify with changing demographics. And the fact that there were 50 million Americans uninsured and 40 million underinsured in the United States. Waste: In the United States, 30 cents of each dollar spent on medical care is wasted, costing $750 billion annually. Inefficient care delivery, high administrative costs, services that aren't needed, high prices, failures in prevention, and fraud all contribute to this. Quality: The U.S. wellbeing framework positions last or close to last contrasted and six different countries like Australia, Canada, Germany, the Netherlands, New Zealand, and the Unified Real on five components of the elite execution wellbeing framework: accessibility, fairness, quality and healthy lives. Hospitals in the United States were found to re-admit an average of one-fifth of Medicare patients within 30 days of discharge, despite the higher costs. According to reports, 25% of Medicare patients discharged to long-term care facilities were re-admitted to the hospital within 30 days, and the list is never-ending. In conclusion, Undeniably, nurses are well-positioned to assume responsibilities that will advance health, enhance care, and increase value. However, it will necessitate new approaches to thought and practice. It is essential to shift your practice's focus away from the disease episode of care and toward promoting health and care across the continuum. Changing Demographics The need for change is also fueled by shifting social and disease-type demographics among our citizens.
