Upcoming Bylaw Implementation: Geographic Requirements for Transplant Hospitals (OPTN Bylaws Appendix D, D.2) Board Approved 12/2016 • Transplant hospitals will receive short member form in early Oct. 2018. • Single donation service area • Under common executive leadership and governance oversight • All ORs within geographically contiguous campus or within 1 mile walking distance – map and addresses required • If do not meet one of geographical requirements in #3, can request an exception

5. Upcoming Bylaw Implementation: Geographic Requirements for Transplant Hospitals • All transplant hospital members will need to complete either: • Geographical Requirements for Transplant Hospitals form • New transplant hospital member application if requesting a merger with another member or splitting into two members • Please notify UNOS Membership staff as soon as possible if your hospital will be: • Your hospital will be merging with another hospital under one membership • Your hospital will need to split into two transplant hospital memberships • Your hospital will be requesting an exception • An FAQ for questions related to merging members or splitting members will be available

Upcoming Bylaw Implementation: Pediatric Transplant Components - Board Approved 12/2015 • Tentative implementation plan: • Spring 2019 – 30 day notice • Late Spring to Summer 2019 – 90 day application period • Summer 2019 to Summer 2020 or Fall 2020 – Staff & MPSC Review of applications • Board approval June 2020 or December 2020 – Board Approval • Dependencies: • OMB approval of revised applications • Delays in implementation of Geographical Requirements for Transplant Hospitals

7. Pediatric Transplant Components: What members need to know to prepare • Kidney, liver and heart: • Primary pediatric transplant surgeon must • Meet all requirements for primary transplant surgeon • Demonstrate current working knowledge and experience in pediatric transplant • Primary pediatric transplant physician must • Meet all requirements for primary transplant physician • Meet requirements of one of the pediatric training or experience pathways • Lung and pancreas: • Primary pediatric transplant surgeon for pediatric component must meet all requirements of primary transplant surgeon for the program • Primary pediatric transplant physician for pediatric component must meet all requirements of primary transplant physician for the program
