Mission Statement As the major source of fundraising for Westfield Memorial Hospital, the Westfield Memorial Hospital Foundation will work with the hospital in providing financial support for the delivery of high quality healthcare to the residents of the Chautauqua region. Relationships with friends of the foundation will be continuously nurtured with the goal of establishing a lifetime of giving.     17. 2010 Campaign Goal The Annual Campaign Goal- $180,000 Matching challenge grant: SVHS has agreed to match every dollar raised in excess of $75,000 up to a maximum of $50,000

Funds raised this year will be used towards equipment purchases in ’09. Sources of Funding Annual Campaign WMH Auxiliary Fashion show Pie sale Cardiac Walk Sources of Funding –Slide#18 Read the slide and add these comments: There are many sources of funding for the foundation with beginning our Annual Campaign. The WMH Auxiliary has been a wonderful fundraising partner as well having raised $10,000 this year alone. In this photo you see: Marjorie Fritz, in the blue shirt, president of our Auxiliary, standing next to corresponding secretary, Deb Mason, who is standing next to immediate past president Marilyn Hemmer in white who is next to auxiliary member, Peggy Sauer. It should be noted this is the sixth consecutive year that the Auxiliary earned a HANYS Auxiliary Advocacy Award in NYS. The Aux holds many special events like an annual fashion Show and an annual pie sale at the Westfield Arts & Crafts Show where the Aux. sold close to $5,000 in pies. Other special events at the hospital included the 21st annual Dr. Robert E. Storms Cardiac Walk for Wellness, which brought in close to $4,400 this past fall. The 16th Annual Golf Tournament was held on July 11 where we raised more than $36,000 tripling our proceeds in 2006. The first signature event for our foundation was a highly successful auction entitled ‘Horsin Around for WMH’ held with our partner, Food is Good, Inc. this in September of ‘06 at the Miller Bell Tower on the grounds of the Chautauqua Institution, where we raised more than $21,000. Sources of Funding (continued) Spencer Legacy Society Endowment Foundations/Grants SVHS This little boy is the son of one of our associates who is sitting on one of the famous cast iron NUFENLAND dog, named either Cantor or Sailor who stand guard over ALL who enter our hospital. One becomes a member of the Spencer Legacy Society after naming WMH in their will. We presently have a $49,000 endowment at the NCCF located in Dunkirk. We get funding from foundations through grants; in fact the one we are most proud of is the $50,000 grant from the Carnahan-Jackson Foundation for renovations to the Emergency Department.     30. $22,000 Balance Master for PT Roxanne Helman, PT assistant stands near our new Balance Master – WE are the ONLY hospital in Chautauqua County to have this piece of equipment which is used by patients with walking and balance Issues; predominantly STROKE patients.     35. New Ultrasound Machine $90,000 Slide # 34 This new state-of-the-art equipment cost close to $95,000 and was purchased with a NYS grant. It is used for a variety of reasons.
