    4. Where do you start? Enrollment Agent: 1-877-223-7781 Brad Robinson Springfield, IL Ext (3000) or Quintin Bradley Tinley Park, IL Ext (4005) If a personal support worker works for multiple participants, each participant constitutes a separate job / Separate Employer A Personal Support worker must ask the Employer to submit an employee packet (for each Employer/Participant they work for)

    5. Service Authorizations

    6. Show me the Pay Rate Table

    7. Multiple Rates A PSW can only earn one hourly rate per consumer, even if performing different tasks A PSW earning $15.00/hour on Friday earns $15.00/hour on Sunday as well

    8. 1. A single PSW total monthly wages, with mandatory taxes, cannot exceed $1941.98/1267.98 Service facilitator fess have to be factored in. Normally 2 hours at a rate of 40.01 and hour for a total of 80.02. 3. The rate [including the mandatory taxes] times the total hours for the month cannot exceed the Participants available budget [p.s. double check the MATH !!!]

    9. SA Continued 4. Dont forget the unit rate must include the mandatory taxes (i.e. 10.5%) 5. Changes to Service Authorization can ONLY change on the 1st of a month [but dont forget specify the MONTH] 6. Changes MUST be received prior to the 4th of the month/

    10. SA Continued 7. Dont forget to include the Recipient Identification Number), RIN. 8. The Social Security number for the participant must be included 9. The Social Security number for the PSW must be included 10. Be certain that the SS# for all of the PSWs AND the participant are correct

    11. SA Continued 11. Please follow the EXACT instructions, e.g. The PSW listed cannot be the employer/service facilitator 12. There must be at least one PSW 13. The Participant/Employer and Service Facilitators signatures make THE Service Authorization official and binding, when all other information is correct 14. You also need the SA signed by the employer

    12. SA Continued 15. Please remember, the SA is YOUR responsiblity. It cannot be submitted by a participant/employer without your signature. 16. Dont forget Minimum Wage law prevails, presently $7.75/hour in Illinois. It will be going up to $8.00/hour in July.

    13. ACES$ will contact Employer Participants who are over budget, however, this will certainly delay payment Like any employer, it is the responsibility of the participant/employer to know exactly how many hours their PSWs can submit and be within their part of the monthly budget

    14. Corrections on Timesheets

    15. White Out ACES$ is unable to accept timesheets that have white out on them, without signed Employer initials at each white out section

    16. Corrections on Timesheets

    17. Overtime The Illinois Department of Labor has determined that overtime shall not apply to domestic employees

    18. Duplicate Timesheets There are over 500 duplicate time sheets every pay period! If an Employer or their PSW submits multiple copies of the same timesheet, those timesheets will be kicked out of our system and will be manually researched---a LOOOOONNG process This process takes time, and will, in most cases, result in a delay of payment We are asking the Employers to instruct their PSWs to submit timesheets only once

    19. Background Checks If a Personal Support Workers State Police Criminal Background check comes back with a record that is on the qualified conviction web site, the onus is on the PSW to obtain a fingerprint test. We have a total of 90 days to accomplish this. NO ONE will start working until CANTS is received by us. If a PSWs CANTS comes back with a record, DHS rules state that they will be unable to work for the participant (IT takes a maximum of two weeks for CANTS Processing) If ACES$ receives notification of a criminal or child abuse record, ACES$ will communicate to the consumer and the worker that said PSW is unable to work in this program.
