Is potential exposure > 50v? No Yes Can an electrically safe work condition be established (i.e. lockout/tagout)? Yes No Is the task considered to be diagnostic/ troubleshooting? No Yes Yes No Coordinate shutdown (live work). Follow lockout tagout procedures. Follow safe work practices and complete the task. Initiate Energized Electrical Work Assessment. Live work is not justified according to established criteria. All 3 levels of review were not obtained. Live work is justified and all 3 levels of review were obtained. Request DENIED.
