What this was • Each year we complete a Self Assessment, this is now a Joint Health & Social Care Self Assessment . • Again this year the form was different, it required social stories as evidence and had more data questions. • The HSA will tell the Department of Health how well health & social care services in our area work together for people with a learning disability.

  3. What is working well (green) • Effective Transitions for young people – we already have clear transition services, however the Transitions Protocols are currently being reviewed and a draft local offer has been created in Milton Keynes. • Effective Joint Working – our formal partnership agreements and arrangements between health and social care organisations are functioning well. • Mental Capacity Act & Deprivation of Liberty – All our providers have these policies in place & regularly monitor & improve practice.

  4. Local Authority Strategies in relation to the provision of support, care and housing are the subject of Equality Impact Assessments and are clear about how they will address the needs and support requirements of people with learning disabilities. • Arts and culture - lots of great examples in Milton Keynes, e.g. Encompass Theatre Group & Inter-Action MK. • Sport & leisure – again lots of great examples, e.g. Freewheelers & MK Dons.

  5. Supporting people with learning disability into and in employment – supported employment scheme, Equality Works. • People with learning disability and family carer involvement in service planning and decision making including personal budgets – via the Learning Disability Partnership Board and all its sub-groups.

  6. What is working OK (amber) • Screening (Cervical, Bowel & Breast) numbers of completed health screening for eligible people with a learning disability. • NHS commissioned primary and community care: Dentistry, Optometry, Community, Pharmacy, Podiatry, Community nursing and midwifery. • Assurance of Monitor Compliance Framework for Foundation Trusts – commissioners review & monitor.

  7. Offender Health & the Criminal Justice System – LD screening tool now being used in Woodhill Prison by the LD nurses. • Assurance of safeguarding for people with learning disability in all provided services and support – the DH Safeguarding Adults Assurance (SAAF) framework goes to everyone. • Regular Care Review - Commissioners know of all funded individual health and social care packages for people with learning disability across all life stages and have mechanisms in place for on-going placement monitoring and individual reviews.

  8. Training and Recruitment – Involvement – we use Experts by Experience for site visits, staff training & during interviews. • Commissioners can demonstrate that providers are required to demonstrate that recruitment and management of staff is based on compassion, dignity and respect and comes from a value based culture - we have LD specific provision, with regards to our Safeguarding & Person Centred Planning Focus Groups aimed at staff working in LD services.

  9. Commissioners can demonstrate that all providers change practice as a result of feedback from complaints, whistleblowing experience – evidence that 50% of commissioned practice & contracts require evidence of improved practice. • Community inclusion and Citizenship – Keeping Safe LDPB sub-group. • Family Carers – all carers are entitled to a carers assessment from Adult Social Care Access Team.

  10. What needs more work (red) • LD QOF register in primary care – to ensure all GP practices are signed up to recording LD data. • Primary care communication of LD status to other healthcare providers – system 1 has not been rolled out yet across Milton Keynes, end of 2013.

  11. Screening (Obesity, Diabetes, Cardio vascular disease Epilepsy) – more data required. • Annual Health Checks and Annual Health Check Registers – practices need to give access in order to validate registers & Annual Health Checks need to be carried out.

  12. Health Action Plans are generated at the time of Annual Health Checks (AHC) in primary care and these include a small number of health improving activities. Refer to RCG guidance around health action plans – Health Action Plans need to be recorded.

  13. Learning disability liaison function or equivalent process in acute setting: - No designated LD liaison function in Milton Keynes.

  14. What next? • The arrangements for this year’s SA differ from previous years, as such there is no validation process but a quality assurance process. We are waiting on clarification from the Department of Health about how this will happen in local areas. • We will create an action plan from this years SA outcomes to improve things for 2014. • When?
