  2. The reactor had a high positive void coefficient at the time of the event

  3. Plant procedures and training did not adequately address low power op’s

  4. The 9 hour delay at 50% power changed the situation

  5. Starting the test caused a reactivity transient

  6. When the operators saw power increase they scrammed the reactor

  7. The scram initially inserted more reactivity

  8. The safety analysis was inadequate or not communicated

  9. The test was not reviewed from a nuclear safety perspective

  10. Operating experience from other plants was not communicated
