  1. Nursing Admission History with New Med List Tool How To Use the Med List Tool (MLT) Step 1. From the Nursing Admission History, click Edit to open the Med List Tool and document the p Delete The Medication Summary screen displays with status and date/time list collected Add a medication (denote last dose take in comments) Left column pulls previous VUMC medication list from Patient Summary (from clinic visit, last hospitalization, etc.) NON EDITABLE Right column is “SCRATCH PAD” with VUMC list of medications EDITABLE Step 2. Edit, Deleted and Add medications Edit MLT is in section 1 & should be completed within 2 hours of admission Step 3. Click “Submit for Review” when all edits are completed

Supporting Code Status Indicator Changes for Nurses What to Look for ( in order of priority) UD- indicating that pt had a previous DRN order but that has NOT been entered in the electronic Code Status note for this admission. NOTIFY ATTENDING . CSLs and RSLs are responsible for monitoring the Indicator and contacting the Attending if the Indicator shows an Attending Signature is needed on Code Status Instructions.  D – indicating that the attending has signed a DNR/ DNI order/documentation UF= previously had a full code documented but that has NOT been entered in the electronic Code Status Note for this admission F= full code The Attending completes the Code Status Form in Starpanel to document/ initiate orders . This must be done with EVERY admission- DNR orders don’t carry over from admission to admission This documentation generates the orders and hence the indicator on the whiteboard
