Accident Investigation An Employer should immediately investigate the cause of any accident or other incident that : • resulted in injury to a worker requiring medical treatment • did not involve injury to a worker but had the potential to cause serious injury • Involves property damage or injuries to third party

  3. Accident Investigation Make sure employees/supervisors have written procedures, training, and accessibility to complete accident investigation forms. Accident packets Cameras Instructions/phone numbers Emergency evacuationplan

  4. Purpose of Investigations • to gather information • to make recommendations • to implement corrective measures

  5. The Investigators The Investigators: • Manager/Supervisor of area • Safety Professional • 1- 2 Employees who are knowledgeable in the work involved **Should have training and tools**

  6. Conducting the Investigation Manager/Supervisor receives the accident/injury report form Ensure any immediate hazards are removed Get the Investigators on the case

  7. Conducting the Investigation Get the Investigation team verbal and written Accident Information ASAP Conduct Investigation in a Timely Manner. Identify any concerns noted about the incident ***tools?***

Investigation Tips and Guidelines • Gather the information: • Speak to the people involved • Determine exactly what happened • Analyze the information: • List The sequence of events • Determine the cause(s) of the incident • Identify root cause and contributing factors • Make recommendations • Focus on prevention of a repeat incident • Follow-up to ensure recommendations are implemented and effective

(i.e. gloves) • Are there safe work procedures written for the safe use of the chemical? • Does the worker have adequate Hazard Communication training?

Summary Checklist • Immediately review accident report, notify the Investigator team • Remove any immediate hazards or dangers that have the potential to cause injury • Initiate the Investigation process ASAP • Immediately follow-up on recommendations made and implement corrective measures • Ensure corrective measures are effective in preventing a similar repeat incident in future • If you have questions or concerns about your Investigation procedures, contact your Summit Loss Control Consultant.
