  2. Steps to enter older adults….

  3. Select an older adult who is… • Aged 65 years or older • Living independently in a home setting • Experiencing some difficulty in performing at least one activity of daily living

  4. Identify a person surrounding the older adult who… • Cares about and for the older adult in some manner (spouse, adult child, friend, neighbor, designated church volunteer) • Provides the older adult with some services – usually related to an activity of daily living – that help the older adult to maintain independence in a home setting (“care giver”)

  5. Complete the one time (annual) assessments… • Functional abilities of the older adult • Demographic characteristics and abbreviated medical history of the older adult • Care giver life change • Care giving stress assessment

  6. Make a file of the assessments on your work computer… • Use the electronic copy at the NOAH link (http://www.fcnntc.org/programs-projects.html#NOAH) • Keep your records either in this computer file, or hard copy • Email to Joann Wessman as an attachment (jwessman@earthlink.net)

  9. Expect the entering stipend of $80… • Within 2 months (currently checks are issued the first of November, January, March, May, July, and September) • Check made out to the church of the participating FCN • Check sent to the attention of the FCN (unless otherwise stipulated) $80

  10. Maintain some form of monthly contact with the older adult and/or care giver… • Visit • Conversation/observation at church • Report from pastor, lay volunteer, etc. • Email • Phone call • Other email monthly report to Joann

  11. At year anniversary month of older adult’s entry into NOAH… • Reassess older adult and care giver and email to NOAH • Expect the $80 stipend for submission of monthly reports in the first year and for doing the reassessments. • An additional $80 stipend is given for entering the older adult into the second year of the NOAH program (total of $160 at the end of Year One if the older adult continues in the project)

  12. Remember that… • Alterations in initial sequencing of steps is fine. You may choose, for example, to… • Initially present NOAH to an older adult church gathering • Involve the older adult, care giver, or lay volunteer in the initial assessments. • Coaching on the process of entering older adults is available as well as coaching on services to the older adult and/or care giver • You can contact Joann if you have questions on eligibility of older adult for the program by phone (763/754-1152) or by email (jwessman@earthlink.net)
