  3. What is the problem? • Chronic kidney disease (CKD) • Affects 31 million people in the U.S. • Permanent damage to the kidneys • Increases a person’s risk for • Heart disease • Anemia • Bone disease • Stroke • Usually no signs or symptoms

  4. Who is at risk? • Those most at risk for CKD: • Have diabetes • Have high blood pressure • Have heart disease • Have a family history of CKD • Are over 60 years old • Are African American, Hispanic, Asian or Native American

  5. What can we do? • Raise awareness • Screen • Refer to resources
