MSPD Status • All Mod Spec activities are currently suspended and will resume after the government shutdown ends • Due to the government shutdown, any recommendations which would affect the scope of work cannot be confirmed at this time • Volunteer members will record any recommendations and provide to ONC once the government shutdown ends • All representatives presenting today are volunteers

Modular Specifications Provider Directories (MSPD) Overview • Mod Specs efforts are aimed at identifying existing specification for a particular area of interest and improving/documenting/testing/promoting that specification • Open process • Public calls held throughout artifactdevelopment • Formed, consulted an Advisor Group consisting of : • Robert M. Cothren Technical Director, California Health eQuality Institute for Population Health Improvement | UC Davis Health System • Walter Sujansky Independent Consultant, Health Information Technology and Clinical Data Integration and Interoperability • Vince Lewis Principal Architect at GSI Health • Gary Tiechrow Co-Founder, Chief Architect at Mirth Corporation • John Donnelly President, IntePro Solutions Inc.

MSPD Project Goals • Develop Implementable, Testable, Certifiable: • Requirements Traceability Matrix (RTM) • Implementation Guide (IG) • Create a Test Implementation (TI) based on RTM • Available for download • Create quality test cases • Available for download (SoapUI) • A tool that can test conformance to the RTM

MSPD Environment Scan Results • MSPD effort based on IHE HPD Specifications • Extend HPD based on feedback from pilot communities such as NATE (formerly WSC), CAHIE and others to include: • Federation Facilitation • Absence of a centralized authoritative provider directory requires federation capability (within and across organizations) to use existing authoritative directories so that electronic addresses of providers and their systems can be discovered during workflows • Error Handling • Provide ability for a client to implement local policies and workflows based on complete/incomplete responses received • Adding federation and error handling will allow consistent and interoperable usage of IHE HPD profiles where federation is required instead of one-off solutions

Proposed Changes to existing IHE HPD • Allow Provider Information Consumers (requestors) to indicate to a Provider Information Directory that a request needs to be federated to other known directories. • Allow Provider Information Consumers (requestors) to receive responses that clearly identify the Provider Information Directory that produced the result and/or errors. • Allow Provider Information Consumers (requestors) to specifically target a Provider Information Directory for subsequent queries based on initial query results. • Allow for clear distinctions of errors that occur due to web service failures versus errors that occur due to data related issues at the Provider Information Directory. This improves the error handling and also debugging and implementation of the specification in the real world. • Provider Information Consumers will be able to detect partial versus complete responses based on the improved error handling and packaging of federated requests and responses. • Provider Information Consumers will be able to identify the authority of the source so that they can log the information for HIPAA and other compliance issues.

  7. Existing Artifacts and Discussion • ONC MSPD project created a solution and vetted with community members in the US to address federation and error handling • ONC Solution is available to jump start activities • http://modularspecs.siframework.org/Provider+Directories+Artifacts • All ModSpec artifacts available for the above solution • RTM • IG • Test Cases • Test Implementation that can be used as a reference • Conformance Test Tool • Estimated Size of Effort = Small (Based on the ONC Mod Spec Activity) • HIE’s within US dealing with the problem currently as identified by pilot efforts by ONC and NYeC
