OEF OIF OND Program Office • In the Spring of 2007 the Department of Veterans Affairs created the OEF/OIF/OND Program Office to expand services to veterans as they transitioned from the military into the VA Healthcare System. • Coordinated Care Management Structure for Seriously Injured/Ill and Non-SeriouslyInjured/Ill OEF/OIF/OND Veterans. • Timely, quality, seamless and well-coordinated care management services to address physical, mental, and psychosocial needs of Veterans. (Office of Patient Care Services, Care Management and Social Work Services)

  3. Expanded Healthcare • On 28 January 2008 the National Defense Authorization Act of 2008 was signed into law by President Bush. • OEF OIF OND Combat Veterans now have free deployment related healthcare extended for 5 years (from the date of discharge) at all VA Medical Centers. • OEF OIF OND Veterans have 180 days to register for free dental care from the date of discharge on their DD-214

  4. Three Core Services: 1. Transition and Reintegration 2. Care Coordination and Advocacy 3. Proactive Outreach/Education/Training

  5. Transition & Reintegration Goals: • Honor and serve our OEF OIF OND Veterans through enrollment in the VA. (Receive 5 years of cost-free health care for any injury or illness associated with their service). • Participate in DoD Demobilizations, Post Deployment Health Re-Assessments, and other local military activities. • Partner with Military Treatment Facilities and VA liaisons.

  6. Care Coordination & Advocacy • Assess and provide appropriate level of case management services to every enrolled OIF/OEF/OND Veteran (Intensive, Progressive, Supportive, Lifetime). • Work with all VA, DoD, and community health care partners to address Veteran’s bio-psychosocial needs. • Provide advocacy services to individual Veterans to address concerns and reduce system barriers to seamless care.

  7. Outreach, Education and Training • Collaborate with all VA/VHA program staff, community partners and Veterans/family/significant others to improve VA services and Veteran health care outcomes, (Focus Groups). • Proactive outreach and collaboration with CBOC’s, Vet Centers, Veteran Service Organizations, law enforcement, community providers, and businesses/employers. • Welcome and educate Veterans, their families and the community regarding VHA services for OEF/OIF/OND Veterans (Welcome Home Event).

  9. Program Manager • Administrative and clinical responsibility for the Program. • Ensures all OEF OIF OND Service Members and Veterans receive patient-centered, integrated care.

  10. Transition Patient Advocate • Information about health care and Veteran benefits. • Assist Veterans in understanding rights and responsibilities. • Resolve complaints, breaks down barriers. • Outreach coordination.

  11. Case Managers - RN & LCSW • Assessment • Individualized care management for readjustment needs • Supportive counseling • Assistance with access to VA, community services and resources

  12. Presenting Needs & Concerns • Physical Health • Signature Injury: Traumatic Brain Injury (TBI) (Motor Vehicle Accident [MVA], Improvised Explosive Device [IED], falls, blast exposure).   14. PTSD Symptoms • Startle reactions to noise or movement • Reliving of traumatic memories • Guilt and/or depression • Concentration & memory problems • Sleep problems, dreams & nightmares • Alcohol and/or substance abuse • Authority problems • Anger Management

  15. TBI – Traumatic Brain Injury • Traumatic Brain Injury is the signature wound of the war in Iraq & Afghanistan. If you as an OEF/OIF veteran were exposed to IED blasts, mortars, RPG’s, accidents, or other injury where you were dazed, confused, or lost consciousness and are experiencing any of the following symptoms please call us or walk-in to our offices. We can screen you for further evaluation & referrals.

  16. Traumatic Brain Injury Symptoms • Frequent or continuous headaches • Ringing in the ears • Dizziness or balance problems • Irritability • Memory Problems • Sleep Problems • Sensitivity to light or noise

  17. Presenting Psychosocial Issues & Concerns Relate to: • Primary support group (Spouse, family of origin) • Interaction/functioning in social environment • Finances • Vocation/Job • Education • Housing/Homelessness • Legal • Accessing/Navigating health care services
