  2. Click to create a new Human Ethics application for a brand new study

  3. Page 1 of the Application Begin by selecting the Principal Investigator. If you have previously held a PI role, this field may automatically be populated with your name. Guidance Notes are located in the gray boxes to the right of the application form. They run the length of the application and include detailed explanations of the questions, instructions on how to fill out the form and useful links to documents and contacts

  4. Filter by the last name and include a “%” in front of the keyword to maximize results Select the name of the Principal Investigator Click “Ok” to add your selection

  5. Make it a habit to hit “Save” before you “Continue” to the next page Please take notice that some questions are marked with a red asterisk and indicates that the question is a required field. If you leave these questions unanswered, you will not be allowed to proceed to the next page of the application

  6. Page 2 of the Application Whenever you encounter questions that require you to input a date, please use the calendar icon to select your dates as it will ensure the proper formatting of your entry.

  7. Page 4 of the Application You will encounter questions where you must click “Add” to select an item from an established list

  8. v p for “VCHRI” For “Providence Health Care” Etc… Option 1: Search for the first letter of the institution Option 2: Simply leave the field blank and click “Find” for the complete list

  9. Click “Ok” to add your selection Select one or more Institutions and the corresponding sites where your research will take place

  10. Page 9 of the Application Attach documents into the body of your application by clicking “Add”

  11. RISe Tutorial Protocol Name your document Include the document version number 1 February 12, 2007 Select a version date Browse your computer for the document you want to attach Click “Ok” to add your selection

  12. Last Page of the Application “Save & Close” out of the application – You will automatically be taken to the Study Homepage

The Study Homepage As the state of your application is “Pre Submission”, your application is still open for any edits you may want to make – click on the “Edit Application” button to go back into the application. To track the status of your application through the approval process, refer to the “Current State” box A newly generated Study ID Number Your Study Nickname Activities that you may perform

  14. Click “Save” as you navigate between pages Click “Exit” to return to the Study Homepage Use the Jump to: tool to navigate the pages of the application

  15. Click to submit your new application for review

  16. Select the appropriate department for the review and approval of your application Click “Ok” to send your application

  17. Notice the state of your application is now “Department Review” and your application is awaiting approval from the Head of the Department of Medicine All your activities and comments can be viewed under the Correspondence tab
