How to report liability claims… • All incidents and accidents (regardless of magnitude) must be immediately reported to two (both) insurance companies: Chubb and Lexington – following the instructions in the next slides • Loss reporting is available 24/7 • For any incidents or accidents that occurred before 7/1/08, please immediately contact Joe Cianciulli/Marsh at 215-246-1261 or joseph.cianciulli@marsh.com

  4. 1) Preparing the loss/claim report All general liability and automobile claims should immediately be reported to (both) 1) Chubb and 2) Lexington. Prior to reporting the incident, accident or claim, please review your records to ensure the following information is available: • Location Name and Code: SMG FACILITY--–# ___ • SMG location • SMG contact (name, full details) • Specific location of incident or accident • Date of incident or accident • Patron(s) name, address and telephone • Description of injury or damage • Witnesses: name, address, and telephone • How the incident or accident occurred **LOSS REPORTING INFORMATION** > For incident with dates that fall between 7/1/08 - 6/30/09 1) Chubb: General Liability - Policy No: 79544602 Automobile: All states except Virginia and Hawaii - Policy No: 79544601 Virginia only:Policy No.: 73528350 Hawaii only: Policy No.:79544755 2) Lexington: Policy No.: 304-4545 (this applies to both General Liability and Automobile) > For incidents with dates that fall between 7/1/09 - 6/30/10 1) Chubb: General Liability - Policy No: 79544602 Automobile: All states except Virginia and Hawaii - Policy No: 79544601 Virginia only:Policy No.: 73528350 Hawaii only: Policy No.:79544755 2) Lexington: Policy No.: 021404546 (this applies to both General Liability and Automobile) ______________________________________________________________________ NOTE: SMG does not release incident reports unless demanded by plaintiff’s attorney or by order of the court. DO NOT have a patron sign an incident report. This will surrender SMG’s right to keep the incident report confidential.

  5. 2) Reporting the loss/claim1) to Chubb There are 3 reporting options available: 1) Reporting by telephone: Call: 1-800-252-4670 and provide the information required under “**LOSS REPORTING INFORMATION**” (outlined in slide #4) 2) Reporting by facsimile: 1-800-300-2538 and provide the ““*LOSS REPORTING INFORMATION**” (outlined in slide #4) or (THE FOLLOWING IS THE PREFERRED METHOD OF REPORTING) 3) REPORTING VIA INTERNET -Go to www.chubb.com -Click on Report a Loss -Click on Claim Notification page next to U.S. Claims -Select Automobile or General Liability -Fill in the appropriate information Information can be pre-set by putting a check mark next to: -Preparer Information -Insured Information, which will always be: SMG 300 Four Falls Corporate Center 300 Conshohocken State Road, 4th Floor Suite 450 West Conshohocken, PA 19428 -When you have completed the electronic form, click on Submit and Print - a Chubb Submission Confirmation will appear when claim is accepted.

  6. 2) Reporting the loss/claim2) to Lexington There are 2 reporting options available: 1) Reporting by e-mail: lexingtonexcessfnol@aig.com and provide the information required under “**LOSS REPORTING INFORMATION**” (outlined in slide #4) 2) Reporting by facsimile:866-244-0519 and provide the information required under “**LOSS REPORTING INFORMATION**” (outlined in slide #4)
