Child Profile Purpose • Provide child health information in an integrated presentation • Support the provision of services through the medical home • Identify preventive services that have been provided and are needed • Eliminate duplicative services • Coordinate private and public health activities • Support the provision of services by coordinating public health programs

Confidentiality • Access to Child Profile Information is role based • State law allows for the sharing of immunizations, hearing and lead information to persons undertaking the care of the child • Mothers sign a consent form for the release of dried blood spot information • Only private and public health providers will have access to the system

  6. Future Plans • Add information on case managers • Add last known physician • Add update capability • Make accessible to physicians, emergency rooms, school nurses, public health programs, etc
