  3. What is CHARM? • Connects specific UDOH program databases so that a health care provider can track, monitor and access information about a child such as: screening results, immunization status, assessments, services received, and referrals

  4. PRIMARY GOALS • To create an electronic virtual “health profile” for children in Utah • Allow real-time access and data sharing among appropriate health care programs, partners, and state agencies

  5. CHARM INTEGRATION(current) • USIIS – Immunization Registry • Vital Records • Newborn Hearing Screening • Baby Watch/Early Intervention (BTOTS) • Office of Recovery Services (DHS)

MEDICAL HOMEPORTAL • Medical Home Portal (MHP) - Information, tools, and resources to aid primary care physicians in caring for Children with Special Health Care Needs and providing a Medical Home for all of their patients (www.medhomeportal.org) • Medical Home is an approach to providing comprehensive primary care

RecordManagement AdministrativeServices InformationServices WHAT CAN CHARM DO? • Assign unique client identifiers • Store and manage core demographics • Track and manage shareable child health profile • Identify and merge duplicate records • Map identifiers across participating programs • Broker for Child Health Profile information • Compile information so it is easy to interpret • Enable reminders and alerts • Maintain user profiles • Enforce user access, security and authority • Maintain an audit trail • Monitor performance
