  8. Bjarne Bjarneis 58 years old and married. He has been diagnosed with diabetes 5 years ago. Bjarne has had a tough time adjusting to a life with diabetes, and has never put an emphasis on a healthy life style. Bjarne will end up costing the health services a great deal of money if he remains passive and is not offered prevention.

  9. Bjarne prepares for a check up Bjarne is summoned to a regular check up at his GP, and uses his netbanking id to access the eHealth portal and be reminded of the goals of his treatment. See what Bjarne finds

  10. Bjarne at the GP Bjarne’s GP access data that benchmarks Bjarne’s progress with other diabetes patients in Denmark. See what the GP finds Bjarne’s numbers show that he does not exercise enough and has more sugar in the diet than recommended.

  11. Bjarne at the GP Bjarne is motivated to start living healthier. Bjarne is referred to a patient education course in his municipality. Bjarne’s GP finds a relevant course for Bjarne in the cross-sectoral list of health and prevention programs offered by municipalities or hospitals. See what the GP finds

  12. Bjarne is given answers and suggestions Bjarne’s GP receives the results of the blood test 2 days later, and shares the results with Bjarne via a secure email. The GP adds a link with diet advice from the Doctor’s Handbook to the email. The GP reminds Bjarne that his next check up is at the hospital. See the link to diet advice

  13. Bjarne’s yearly check up at the hospital Bjarne asks a question regarding the relationship between his wellbeing and his diet. The doctor at the hospital access his Bjarne’s Medicine Profile with prescriptions to check if his medicine can explain his mood swings. See what the doctor finds

  14. Has his vision deteriorated? Bjarne is reminded that he should have his vision checked, since vision can be impaired as a consequence of diabetes. Bjarne has tried calling his ophthalmologist, but he has retired and the clinic is no longer open. The hospital doctor suggests that Bjarne should find a new ophthalmologist What does Bjarne do?

  15. His eyesight is worse Bjarne finds a new ophthalmologist in his area in the national directory of the eHealth portal. See what Bjarne saw before he chose The ophthalmologist discovers that Bjarne’s vision has deteriorated and that it is time for laser surgery.

  16. Laser surgery The operation is a succes and improves Bjarne’s sight. Bjarne remembers that he is intolerant of the eye drops he was prescribed an earlier time, but not which eye drops. The ophthalmologist wants to make sure that Bjarne uses eye drops, and accesses his Medicin Profile to avoid using the same eye drops. See how the ophthalmologist found the name of the eye drops

  17. Bjarne feels better The improvement of his sight helps Bjarne lighten up, and he is very motivated as he joins the patient education course of the municipality. Continue til main presentation
