  12. UP Campaign: Spreading Cross Cutting Strategies Designed to reduce multiple forms of harm with simple, easy-to-accomplish activities that cut across several topics to decrease harm. Focused on four components: • SOAP UP: Hardwire Hand Hygiene • GET UP: Mobilize Patients • WAKE UP: Prevent Over-sedation • SCRIPT UP: Optimize Inpatient Medications 12

AHRQ Patient Safety Program for ICUs: Objectives and Overview • 12 month cohort beginning January 16, 2019 for eligible hospital Adult ICU’s focused on reducing: • CLABSI • CAUTI • Eligibility to Participate • + CAD in the TAP Report • Informational Webinar • December 6, 2018, 1:30-2:30pm • Enrollment Deadline • December 14, 2018 14

AHRQ Patient Safety Program for ICUs: Benefits • Support in achieving Department of HHS 2020 CLABSI/CAUTI reduction goals • Gain experience with implementation of the Comprehensive Unit-based Safety Program (CUSP) to create a “culture of safety” • Access to Resources • Educational materials • Subject Matter Experts • Coaches • Benchmark reports • Peer learning and sharing of “best practice” 15
