  4. Step 3 1. To access the notes Click on the Catalog tab within PowerNote 2. From Catalog Dropdown Tab Choose Home Health Or Choose Home Oxygen Assessment 3. If ordering both Home Health and Home Oxygen, you must complete a PowerNote for both.

  6. Face to Face Documentation 2. Complete Clinical Reason/Diagnosis for Home Health i.e. Diabetes, Wound Care Must include clinical diagnosis 1. Choose Date and Time from Calendar 3. Choose Type of Service Please include Skilled nursing for medication management 4. Choose One or More of the Home Care Interventions 5. Select Two or More of the Homebound Reasons along with diagnosis to support. ( post op weakness, Exacerbation CHF, Debility. Theremust be 2 homebound qualifiers 6. Choose an Outpatient Managing Physician Select from the Provider List 7. Must Click to Certify and Sign!

  7. PowerNote is Complete! When Certification field is clicked, documentation is added to Physician’s Progress Note in Patient’s Care Connection Record once you click sign and submit. F2F power note must be cosigned by an attending.
