  2. PART III- RECORD OF OBSERVATIONS AND COUNSELING a. SUMMARY OF OBSERVATION: Summarize most significant observedleadership behaviors. Use sufficent detail to support ratings in Parts I and II. Use continuation card if necessary. CDT XXXX performed in an excellent manner as PRT from 230600JAN12 to 230700JAN12 . CDT XXXX communicated effectively by getting with his evaluator prior to his pt session, and notifying all AI’s of his rehearsals (CO). He maximized training and utilized MSI’s and MSII’s as AI’s and asked for their input on his pt plan (DO, IP).

PART III- RECORD OF OBSERVATIONS AND COUNSELING • Abbreviate Cadet to CDT • Saves space • Keep sentences in the SAME tense • Write complete sentences • Include time of session and date • EX. 270600JAN12 to 270700JAN12 • You are observing the Cadet’s actions • Summary of observation (what they did) • Avoid or DO NOT write sustains or improves • You can have negative comments in your summary of observations

  4. PART III- RECORD OF OBSERVATIONS AND COUNSELING b. COUNSELING: Comment on at least 2 “SUSTAIN” and 2 “IMPROVE” attribute and/or core leader competency as identified in Part 1 and Part II. (“IMPROVE” comments are required for each “N” entry in Part I and II) Not required for Spot Report. CO- Utilized his AI’s in his decision making process by allowing them to have an input in his PT plan; added more exercises PS- Conducted 2 rehearsals to ensure a PT plan SJ- Down graded the PT uniform based on the weather DO- Failed to correct Cadets when performing an exercise incorrectly RS- Failed to recover quickly from shock or stress when he was missing PT materials to conduct his workout

PART III- RECORD OF OBSERVATIONS AND COUNSELING • Comment on at least 2 “SUSTAIN” and 2 “IMPROVE” • Focus on Attributes and/or Core Leader competency • Leader of Character, Presence, and Intellectual capacity • An Army Leader leads, develops, and achieves • Answer how they did and what they did to receive that leadership dimension • Abbreviate Leadership Dimensions • DO NOT use the leadership dimensions from Part III a.

YELLOW CARD: Important • DO NOT just tell your evaluator what you did • How you did it and why • S.T.A.R. • Situation, Task, Action, Results, and REPEAT • There should be more than 1 Action under a Task • If not then it’s not that important • Abbreviate • BACK Side • List at least 2 Strong/Improve Dimensions • List bullet comments • Abbreviate Dimension • Cadet Summary Counseling • This will be done after you have both reviewed the Blue and Yellow Cardtogether
