  15. How Can A Representative Help? • A Representative may sign agreements for you • A Representative may help you hire and manage workers or providers 15 Self-Determination Peer Project

  16. How Can A Representative Help? • A Representative may sign the timesheets for your workers • A Representative may help if there is a problem 16 Self-Determination Peer Project

  18. 4 SUPPORTS COORDINATORS AND CASE MANAGERS

  19. Who are Support Coordinators and Case Managers? • A supports coordinator is someone from the CMH who is responsible for helping you determine what CMH services and supports you need • Case Manager is the same thing for different programs 19 Self-Determination Peer Project

How Can A Supports Coordinator Or Case Manager Help? • Figure out what you goals are (such as finding a job, living on your own, doing an activity you like) • Identify the services and supports you need to reach your goals • Make sure that you are getting the services and supports you need 20 Self-Determination Peer Project

  23. What is an Independent Facilitator? • someone you choose and hire • to help you with your person-centered planning process • independent from the system • has training in PCP 23 Self-Determination Peer Project

  24. How Can An Independent Facilitator Help: • Run the meeting(s) for you or with you (your choice) • Advocate for you as much or as little as you want 24 Self-Determination Peer Project

  31. 7 FISCAL INTERMEDIARY

  32. How can a Fiscal Intermediary Help? • Pay invoices for your services after you approve them • Pay your workers after you approve their timesheets • Send you a monthly budget report 33 Self-Determination Peer Project

  36. What is an SD Peer? • A person who has been trained as a peer mentor or certified peer support specialist • Who also has training in self-determination • Who can be hired by a person to help him or her with using arrangements that self-determination 37 Self-Determination Peer Project
