Mental Health OutcomesContract Exhibit D/Performance OutcomesSTABLE HOUSING Outcome – Percent of (individuals in specified population group) who live in stable housing environment Definition • Stable housing environment includes private residences with relatives, non-relatives, friends, or foster parents • It does not include institutional settings, residential care, residential treatment facilities, crisis residences, jail, correctional facilities, or homeless • Requires “Residential status code” of 01 thru 08, or 11 as entered on the PERF

Mental Health OutcomesContract Exhibit D/Performance OutcomesDAYS WORKED FOR PAY Outcome (Forensic, SPMI, Crisis) - Average Annual Days Worked for Pay Definition: • Must meet criteria for this pop group • Score is based upon the response given on the form for Total Days Worked in last 30 days • Requires a Purpose Code 2 (Quarterly ) or 3 (Discharge) MH Outcome record within the fiscal year • Requires Service Event within the fiscal year to be included in calculation • Includes any period of time period within a calendar day that results in taxable income • Data set size determined by submission of quarterly/discharge Performance) outcome and service event – both within the fiscal year • Clients 62 years of age & older are excluded

Additional information • Can include paid leave • Volunteering cannotbe counted • Do not report 00 as a matter of convenience as it brings down scores dramatically – Research to obtain valid data • Be sure to accurately report the number of days

Mental Health OutcomesContract Exhibit D/Performance OutcomesCOMPETITIVELY EMPLOYED Outcome: ( Forensic, SPMI, Crisis) Percent of Adults Who are Competitively Employed Definition: • Must meet criteria for one of the designated population groups • Requires a Purpose Code 2 (Quarterly ) or 3 (Discharge) MH Outcome record within the fiscal year • Requires Service Event within the fiscal year to be included in calculation • Employment status must be full time or part time any time during the fiscal year • Employment Status Codes: 1= Active Military, overseas; 20 = Active Military, USA; Full time = 30; Part time = 40

Mental Health OutcomesContract Exhibit D/Performance OutcomesSCHOOL DAYS ATTENDED Outcome: (SED) Percent of School Days Attended Definition: • Must meet criteria for this pop group • Requires a Purpose Code 2 (Quarterly ) or 3 (Discharge) MH Outcome record within the fiscal year • Requires Service Event within the fiscal year to be included in calculation • School days attended are the days on which school was in session and the child attended school Additional Info • Score based upon response to Number of School Days attended in last 30 days on MH Outcome Form • Can enter up to 22 days • Be sure that School Days available is correct as this is used in the calculation • Clients in custody of DJJ are excluded
