  3. WHEN CAN I BECOME REGISTERED? • AFTER COMPLETING THE PHARM D I YEAR, HOWEVER, IF YOU ARE NOT GOING TO WORK WITHIN THE BOUNDARIES OF THE STATE OF MINNESOTA YOU DO NOT NEED TO REGISTER AT THIS TIME.

  4. HOW DO I BECOME REGISTERED? • FILL OUT THE INTERN REGISTRATION FORM • IF YOU CANNOT LOCATE YOUR FORM YOU CAN GET IT OFF OF OUR WEB SITE AT WWW.PHCYBRD.STATE.MN.US

HOW DO I BECOME REGISTERED? • GIVE THE COMPLETED FORM TO THE OFFICE OF STUDENT AFFAIRS, ALONG WITH A CHECK FOR $20.00 MADE PAYABLE TO THE BOARD OF PHARMACY. • THEY WILL SEND THE FORM AND CHECK TO US AS SOON AS THEY VERIFY THAT YOU COMPLETED THE PHARM D I YEAR. • THEY WILL HAVE THE FORMS TO US DURING THE WEEK OF MAY 22, 2006.

  6. WHEN CAN I START WORK? • WE WILL BACKDATE YOUR REGISTRATION TO MAY 14, 2006. THEREFORE, YOU CAN BEGIN WORKING AS A REGISTERED INTERN ON THAT DATE, AS LONG AS YOU COMPLETE THE PHARM D I YEAR.

  7. HOW DO I RECORD MY HOURS? • WITHIN FIVE DAYS OF BEGINNING EMPLOYMENT, TURN IN A NOTICE OF EMPLOYMENT • MAKE SURE THAT YOUR PRECEPTOR IS CURRENTLY REGISTERED AS A PRECEPTOR WITH THE BOARD OF PHARMACY

  8. HOW DO I RECORD MY HOURS? • BY JUNE 15 OF EACH YEAR OR WITHIN 5 DAYS OF A CHANGE OF PRECEPTOR OR SITE, TURN IN A PROGRESS REPORT AFFIDAVIT • MAKE SURE THAT YOUR PRECEPTOR IS CURRENTLY REGISTERED AS A PRECEPTOR WITH THE BOARD OF PHARMACY

  9. WHEN DO I TURN IN NEW FORMS? • TURN IN A NEW FORM ANY TIME YOU: CHANGE SITES CHANGE PRECEPTORS BY JUNE 15 OF EACH YEAR

  10. WHAT TIME FRAME DO FORMS COVER. • NOE forms should begin the day after graduation. • PRA forms should end the day of the following graduation unless you end your employment sooner or unless you change preceptor.

  11. FORMS • WHAT HAPPENS IF I DON’T SUBMIT MY FORMS ON TIME? YOU MAY LOSE INTERNSHIP HOURS. TURN THE FORM IN WITH A NOTE STATING WHY IT IS LATE. IT WILL BE REVIEWED AND A LETTER MAILED TO YOU STATING HOW MANY HOURS YOU WILL RECEIVE.

  12. FORMS • WHAT HAPPENS IF MY PRECEPTOR IS NOT A REGISTERED PRECEPTOR? YOU MAY LOSE INTERNSHIP HOURS. WE WILL CONTACT YOUR PRECEPTOR, WITH A CARBON COPY TO YOU, STATING THEY NEED TO BECOME REGISTERED.

  13. FORMS • WHAT HAPPENS IF I DON’T SUBMIT MY FORMS CORRECTLY? YOU MAY LOSE INTERNSHIP HOURS. IF YOU ARE UNSURE OF HOW TO FILL OUT THE FORMS, CONTACT OUR OFFICE.

  14. HOW MANY HOURS DO I NEED? • YOU WILL NEED 1600 HOURS • YOU RECEIVE HOURS AS FOLLOWS: NON-DISPENSING ADVANCE PHARMACY PRACTICE EXPERIENCE – 800 ADVANCE PHARMACY PRACTICE EXPERIENCE (DISPENSING) – 200 FOR EACH ONE FROM OTHER AREAS – 400 UNLESS YOU ONLY DO ONE ADVANCE PHARMACY PRACTICE EXPERIENCE

  15. WHAT ARE THE TYPES OF INTERNSHIP? • PHARM D I AND II SUMMERS - HOUR FOR HOUR CREDIT SUMMER HOURS FROM THE DAY AFTER SCHOOL ENDS IN THE SPRING UNTIL THE DAY BEFORE SCHOOL BEGINS IN THE FALL. SUBMIT A NOE WITHIN FIVE DAYS OF STARTING AND A PR/A BY JUNE 15 OF FOLLOWING YEAR.

  16. WHAT ARE THE TYPES OF INTERNSHIP? • CONCURRENT TIME – MAX. 400 HOURS TIME THAT YOU WORK WHILE YOU ARE TAKING 12 OR MORE CREDITS DURING A SEMESTER. THE 12 CREDITS INCLUDES ALL CLASSES, NOT JUST PHARMACY CLASSES. SUBMIT A NOE WITHIN FIVE DAYS OF STARTING AND A PR\A BY JUNE 15 OF EACH YEAR.

  17. WHAT ARE THE TYPES OF INTERNSHIP? • OTHER INTERNSHIP TIME COMPLETED (I.E. FEWER THAN 12 CREDITS, WINTER/SPRING BREAK) – HOUR FOR HOUR CREDIT THE 12 CREDITS INCLUDE ALL CLASSES, NOT JUST PHARMACY CLASSES. SUBMIT A NOE WITHIN FIVE DAYS OF STARTING AND A PR\A BY JUNE 15 OF EACH YEAR.

  18. WHAT ARE THE TYPES OF INTERNSHIP? • NON-DISPENSING ADVANCE PHARMACY PRACTICE EXPERIENCE ALL CLINICAL ROTATIONS – 800 HOURS MAX. YOUR EXTERNSHIP COORDINATOR WILL TURN THESE HOURS IN FOR YOU.

  19. WHAT ARE THE TYPES OF INTERNSHIP? • ADVANCE PHARMACY PRACTICE EXPERIENCE (DISPENSING FOCUSED) HOSPITAL & COMMUNITY ROTATION – HOUR FOR HOUR YOUR EXTERNSHIP COORDINATOR WILL TURN THESE HOURS IN FOR YOU.

  20. DO I ALWAYS NEED TO SUBMIT FORMS? • YES, TO RECEIVE CREDIT FOR THE 400 OR MORE HOURS YOU WILL NEED OUTSIDE OF THE COLLEGE CURRICULUM. ALSO, IF YOU WANT TO BECOME REGISTERED IN ANOTHER STATE WE CANNOT TRANSFER YOUR HOURS UNLESS THEY ARE FILED WITH US. IF YOU WANT PHARMACY PRACTICE EXPERIENCE WAIVED YOU NEED THE HOURS ON FILE WITH US AND YOU NEED TO TALK WITH YOUR COORDINATOR ABOUT THIS.

  21. CAN I WORK WITHOUT A PRECEPTOR BEING ASSIGNED TO ME? NO, IN ORDER TO WORK AS AN INTERN YOU NEED TO WORK UNDER THE GUIDANCE AND SUPERVISION OF A REGISTERED PRECEPTOR.

  22. DO I NEED TO REGISTER NOW? • IF YOU ARE NOT WORKING AS AN INTERN YOU DO NOT NEED TO REGISTER UNTIL PRIOR TO BEGINNING WORK. HOWEVER, YOU DO NEED TO BE REGISTERED PRIOR TO BEGINNING YOUR ADVANCE PHARMACY PRACTICE EXPERIENCE.

  23. INTERNSHIP MANUAL • YOU WILL RECEIVE IT IN THE INTERNSHIP PACKET. • HAVE YOUR PRECEPTORS, OR ANY REGISTERED PRECEPTOR AT YOUR LOCATION, SIGN IT WHEN YOU ACCOMPLISH EACH COMPETENCY. • INTERN HOURS WILL NOT BE VALIDATED UNTIL WE RECEIVE YOUR SIGNED MANUAL.

  24. INTERN MANUAL • THIS MANUAL IS A LEARNING TOOL FOR YOU TO WORK ON THROUGHOUT YOUR INTERNSHIP AND PHARMACY PRACTICE EXPERIENCE.

  25. RATIO OF INTERNS/PRECEPTOR • A PRECEPTOR CAN HAVE MORE THAN ONE INTERN ASSIGNED TO THEM; BUT ONLY ONE INTERN CAN BE WORKING UNDER THIS PRECEPTOR AT A TIME.
