Contact Information Information is pre-filled on this form based on the users login information Note that fields that are required have a * beside them.
Order Information Customer number is pre-filled on this form based on the users login, but can be overridden if you need to fill out one for another # 08071960
Order Information Clicking on the load address button will give a choice of address to use for that customer number 08071960
Order Information Select an address radio button to use for that address, Then Click Next NOTE: No PO Box addresses are allowed for parts orders!
Order Information A Purchase Order number is required as freight for parts will be billed.
Add a part(s) Each part on the request requires a serial number, model, part number quantity, and failure reason
Add a part(s) Serial numbers since around 1987 are in the systems with some exceptions. You can enter the serial number and let the system try to fill in the model & size if you wish. Just click this button.
Record of Past Requests Click on this link to view a past request
Ready to Submit Click here if you are ready to submit or “Add A Part” if you need to request another.
Submission Screen If you need to add any special Notes to your request, enter them in the Additional Comments box. If you are finished, Press the Submit button.
Print Part Disposition Tags Click Here to print out the part disposition tags; each completed for you with the date for field scrap- or if the part requires return, that information, a bill of lading and an address label.
