Information may be shared with others involved in providing care such as the young person’s GP or Social Worker, as appropriate. The young person will be provided with written information as regards information sharing, data protection and ECC (Electronic Care Coordination).   2. ‘Bridging the Gap’ We are a new community based service for young people between the ages of 16-19 years in need of mental health services within the borough of St Helens. The team works in partnership with the Children & Young Peoples Service (local authority), with a focus on young people leaving care and children in need. The team has been established to address the mental health needs of young people within this age range, and where necessary assist with the transition from Child & Adolescent Services (CAMHS) to Adult Mental Health Services (AMHS). Currently the team consists of 1x Co-Ordinator 1x Mental Health Practitioner 1x Admin Support The Aim of the Service is To act as a resource for advice, consultation and support to those people working with young people who experience mental health difficulties. Providing assessment/screening and therapeutic interventions on an individual basis or with family /carers. To offer assistance to young people who maybe experiencing difficulties in coping with emotional and mental health issues. To promote collaborative working between services with young people. To engage in mental health promotion and training. • Reasons for Referral • Reasons for referral may include concerns around a young persons mental health that cannot be managed through Primary Care. Problems will be of a moderate nature e.g. depression, anxiety, self –harm and to the extent of disrupting the young persons daily living and normal development. • If unsure, please contact the service. • Making a Referral • Any GP or Health/Social care Professional can make a referral. • The young person must be between 16-19, living in the St Helens area and be known to the St Helens Local Authority Young Peoples Team. • Referrals need to be made in writing to the service or electronically, via the 5 Boroughs referral system. • The young person must be aware of the referral being made. • The young persons GP will be informed of the services involvement with a young person.
