  1. 市售塑化劑污染五大類食品訪視處理流程 (供非屬衛生稽查人員使用) 100.5.31版 注意事項： 1.向店家表明身分(如：志工請佩戴志工證) 2.攜帶訪視表 是否為公告五大類食品 不是 不確定 回報衛生局(所) 符合 是 是否有下列證明之一： 1.起雲劑原料來源證明(未受塑化劑汙染之起雲劑供應商， 如附件一) 2.檢驗證明(六種塑化劑檢驗結果均合格，檢驗機構為衛生 署公布可檢測食品塑化劑之實驗室，如附件二) 3.製造廠商提供未使用起雲劑之切結書、保證書(函)、或聲 明書 有 符合 無 回報衛生局(所)，請填寫訪視通報表 備註： 一、行政院衛生署公告五大類食品包含： 1.運動飲料 2.果汁飲料 3.茶飲料 4.果醬、果漿或果凍 5.膠囊錠狀粉狀之型態(保健食品類) 二、附件一、附件二請於訪視前，至食品藥物管理局起雲劑汙染專區下載確認 為最新資料 三、進口產品不須檢附證明(以產品標示之原產地判定)
