  3. SUCCESSES 2008/9 Implementation of combined retinal/foot screening programme Implementation of Diabetes Local Enhanced Service

  4. CHALLENGES 2009/10 • Continue move to Practice based care(65% plus) • Look to expand Practice based teams/Diabetes education • Skills /knowledge to convert Type 2 diabetics to insulin within primary care

  5. CHALLENGES 2009/10 • NEW Q.O.F TARGETS !!!!! • HbA1C 7 or under > 50% • HbA1C 8 or under > 70% • HbA1C 9 or under > 90% Reduction in morbidity / mortality from diabetes OR • Risks of over-medicalisation, iatrogenic illness and demotivated staff
