  3. Who is being surveyed? All schools and childcare centers in Kentucky must report immunization information for any child who is: • Enrolled in kindergarten or 6th grade • Attending daycare, head start, or preschool (aged 19 through 34 months) DO NOT include children younger than 19 months of age or children enrolled in after school programs.

  4. Role of the School Nurse • Receive the survey, worksheets and instructions from the local health department • Access immunization certificates/information for children in the appropriate grade/group • Complete the worksheet using the instructions provided • Complete the survey using the completed worksheet and instructions provided

  5. Role of the School Nurse cont. • Review the survey to reduce errors before sending • Return the survey to local health department via mail or fax, by • October 29, 2010 • Some schools will be selected for validation by the Immunization Program Field Staff to ensure accurate data is reported.

Reminders • Completing the Survey is required by KY state regulation 902 KAR 2:055 and the CDC. • A separate survey is required for each daycare center, head start program, preschool class, kindergarten class, and/or 6th grade class at each school/facility. • Children younger than 19 months should NOT be included. • Children in before/after school programs should NOT be included (they will be counted in their regular classes).

  12. Helpful Hints • Use the worksheets and instructions that will be sent with the survey. • Check over the final form to ensure that it is filled out correctly before returning it to the local health department. • Never leave any response areas blank. • Remember that the purpose of the survey is to assess number of children with age appropriate doses.
