  2. 2019 Pharmacy Benefits FAQ • Starting January 1, 2019 • All members will receive new ID cards • At the pharmacy, alert the pharmacist that you have a new pharmacy benefit manager – CVS/Caremark • Provide them with your new ID card so they can update your information in their system

  3. 2019 Pharmacy Benefits FAQ • Starting January 1, 2019 • If your medication requires a prior authorization • Your physician will need to contact CVS Customer Care to have a new prior authorization performed to allow your prescription to process • If you are unsure if your drug requires a prior authorization please contact CVS/Caremark at the number listed on the your ID card and ask the customer service rep to perform a coverage check on your medication.

  4. 2019 Pharmacy Benefits FAQ • Starting January 1, 2019 • If you currently have “open” refills on your existing mail order prescription • Any available un-used refills at mail will need to be processed prior to 1/1/2019 • On or after 1/1/2019 a new prescription will be required. Make sure to alert your physician of this change so that a new prescription can be written for your first 2019 Rx claim.
