2. Glucometer Errors Incorrect Health Care Provider Patient ID Not Found i Caused by not inputting a Health Care Provider (HCP) ID or the correct HCP ID after doing the blood glucose test. POCT Errors How to correct your POCT errors “Incorrect Health Care Provider” Login to Essentris 1 Click on “Administration Tab” 2 ✓ Click on “Find Patient” 3 4 Search last four of patients SSAN “*1234” 5 Search both [X] Active [X] Archived files 6 Locate patient in list by matching SSAN and error date. 7 Locate Health Care Provider assigned to patient & annotate.
