  2. Glucometer Errors Incorrect Health Care Provider Patient ID Not Found i Caused by not inputting a Health Care Provider (HCP) ID or the correct HCP ID after doing the blood glucose test. Mostly caused by not scanning patients bar code on identification band and keying input into the Glucometer incorrectly. 1 2 POCT Training Teddie Joe Snodgrass

POCT Errors How to correct your POCT errors “Incorrect Health Care Provider” Login to Essentris 1 Click on “Administration Tab” 2 ✓ Click on “Find Patient” 3 4 Search last four of patients SSAN “*1234” 5 Search both [X] Active [X] Archived files 6 Locate patient in list by matching SSAN and error date. 7 Locate Health Care Provider assigned to patient & annotate. POCT Errors How to correct your POCT errors “Patient ID Not Found” Login to Essentris 1 Click on “Administration Tab” 2 ✓ Click on “Find Patient” and follow steps in preceding slide. 3 4 Check Admissions and Transfers Log Book for patient SSAN 5 Focus on error date and try to match POCT SSAN with log 6 Check 24 Hour Report Error Date with last four of pts SSAN 7 Locate patient in CHCS system.   5. POCT Training Your own sub headline Scan Patient’s Bar Code Keying in Patient’s ID Correcting Errors Always scan patient bar code on their arm band. If a patient does not have an arm band, request one from Admissions. Remember to input HCP ID after each test. If a patient does not have an arm band, request one from Admissions. Be extremely careful in keying in patient correct ID information.
