First Time Freshman • Students who have never attended college • Transfer • Students who have attended college(s) • Certificate • May have attended college; however, only the high school transcript is required even if the student graduated from an institution of higher education Degree Seeking

  3. Transient • Students who attend another institution and plan to enroll at Southwest for one or more semesters and then return to the home institution. These students are not eligible to receive federal financial aid. A student wishing to change to degree seeking status, must meet all admission requirements • Non Degree • Students who do not wish to apply for a degree. These students are not eligible to receive federal financial aid. A student wishing to change to degree seeking status, must meet all admission requirements Non Degree

  4. Official High School Transcript with graduation date • TN grads must pass the Proficiency Exam • Test Scores (ACT, SAT, COMPASS) • ACT and SAT scores are only valid for three years from test date • MMR - Measles, Mumps and Rubella records (Only needed if born after 1956 and full-time) • Proof of Varicella– Chicken Pox (Born after Jan. 1, 1980 and full-time) • Complete Hepatitis B Form • Orientation First Time Freshman

Official GED Transcript (450/45) • Test Scores (ACT, SAT, COMPASS) • ACT and SAT scores are only valid for three years from test date • MMR - Measles, Mumps and Rubella records (Only needed if born after 1956 and full-time) • Proof of Varicella – Chicken Pox (Born after Jan. 1, 1980 and full-time) • Complete Hepatitis B Form • Orientation GED

Official transcript from all schools attended • High school transcript (no previous degree) • Test Scores (ACT, SAT, COMPASS) • ACT and SAT scores are only valid for three years from test date • May be required if student did not successfully complete College-level English and Math • MMR - Measles, Mumps and Rubella records (Only needed if born after 1956 and full-time) • Proof of Varicella – Chicken Pox (Born after Jan. 1, 1980 and full-time) • Complete Hepatitis B Form Transfer Student

  9. Separation from any college for a minimum of 4 years • Consult with an Academic Advisor • Must apply before earning 15 degree hours Academic Fresh Start
