Participants • Is the activity appropriate for the developmental age of the participant? • What forms will be required? • What is the nature of the communication? • How will the skill level of the student be determined? • Do students have adequate insurance? • Will they been given instructions regarding rules and responsibilities? How? • What arrangements have been made for transportation?

  3. Staff • Who will be attending? • Do they have special qualifications? • Has the staff discussed and developed an EAP? • Will staff be given specific responsibilities and how will this be communicated? • What safety/emergency equipment/materials will be provided for the trip? • Does staff have training to respond to an emergency situation? First Aid kit?

  4. Volunteers • How many adult volunteers will be attending? • Have they been cleared (CAR, CRC)? • Do any have special qualifications applicable to the trip? • Will they been given instructions regarding their roles and responsibilities? How? • Will volunteers require training?

  5. Instructors • Will instruction take place? • What is the nature of the instruction? • Do instructors have the necessary certification? • Are there special instructions/rules that are to followed after the instruction?

  6. Facility (Outdoor) • Have the natural hazards been identified? • Is the area inspected? • Are there any modifications or restrictions?

  7. Facility (Non-school based) • Have the hazards been identified? • Is the area inspected? • Are there any modifications or restrictions? • Is the equipment inspected and maintained? Standard?

  8. Personal (Activity-related) • Parental permission to use? • Does it conform to safety standards? • Have there been modifications made to the equipment?

  9. Personal (Protective) • What recommendations have been made? • Does the equipment require a standard? Condition? • Has the equipment been modified?

  10. Risk Management Checklist • Communication and forms? • EAP? • Roles and responsibilities defined? • Instructor qualifications verified? • Medical information? Equipment? • Trip plans/itinerary submitted? • Timelines and signatures? • Other?
