Assessing and Designing a Health and Wellbeing Improvement Plan

This external assessment task requires students to assess a service user's health and wellbeing based on a case study. They need to identify factors that affect the person's wellbeing, interpret lifestyle data, and evaluate risks. Students also need to design a plan that recommends actions, sets targets, and considers sources of support, obstacles, and the person's needs. The task covers various aspects of health and wellbeing, such as physical, social, intellectual, and emotional factors.
