Handrail dislodged from stand resulting in dropped object. During operation of a centrifuge pump, a handrail was vibrated out of its holding position and fell approx 10 ft. into the cement pond. (Traction Report # 2010-IR-3388240). Isolation valves.

  1. Handrail dislodged from stand resulting in dropped object During operation of a centrifuge pump, a handrail was vibrated out of its holding position and fell approx 10 ft. into the cement pond. (Traction Report # 2010-IR-3388240) Isolation valves Discharge line connection failed on 90 connected to valve, line fell to ground. (Reconnected after incident) +/- 10ft 10” line from second filter pod to location Incident Summary – Bonicelli 1-35H Drilling Location, January 30, 2010, During normal operations of the centrifuge, the handrail on the centrifuge stand vibrated out and fell into the cement pond. Centrifuge skid is owned, rigged up and maintained by mud contractor. Drilling contractor did not verify that equipment meet the minimum requirements of their SMS, i.e. handrails pinned in place. The handrail weighs approx. 40 lbs and fell 10 ft. No potential impact to personnel. • Contributing Factors • Hand rails were not properly secured to stand. • Stab portion (bottom) of legs are too short and only have about 1 1/2 inches of contact in slot. • Centrifuge being utilized more than usual for LCM project. • Corrective Actions: • WSL advised of dropped object, operations suspended for investigation • Contract Company to put plan in place to mitigate issue from re-occurring. • Make handrails (stab portion) longer at stand • Secure in place • Drilling Contractor to ensure proper contact is performed with rental company equipment/services on location and meets Drilling Contractors’ standards.
