Learning Objectives for This Session and Action Period #3 • Understand and demonstrate the concepts of diffusion and spread to implement successful changes practice-wide. • Understand and demonstrate how to audit systems and processes of care to ensure sustainability of improvements. • Understand how to develop linkages with hospitals, payers, and community resources to support patient self-care and care coordination of high-risk patients, particularly during the transition from hospital to home.
