  2. Tips for Integrating the Evaluation Data into your System of Care • Add modifiers to the existing data collected (e.g. billing codes). • Meet w/ the program Admin and Clinical staff to review the data collection requirements and conduct a workflow analysis.

  3. Tips for Integrating the Evaluation Data into your System Needed Supplies: • Sticky notes • Easel paper • Tape • Markers

  4. Workflow Analysis • Tape 4-5 sections of easel paper on the wall so the edges are right next to each other. • With a marker draw a line across the top and write in the patient/consumer service flow steps along the line.

  5. Workflow Analysis • At each step in the patient/consumer flow list the information collected and who collects it. • Develop a checklist with the data elements that conforms to the patient/consumer flow. • Include the checklist in the process.
