  1. Free Health Care Clinic Volunteer Retired Physician and Dentist Professional Liability Coverage Program A program of the Alabama Department of Public Health Office of Primary Care and Rural Health Carolyn Bern , Coordinator

  2. Purpose To provide civil liability coverage not provided under the Volunteer Service Act

  3. PROGRAM RULES • Retired Dentist trained in General Dentistry • State of Alabama License • Minimum number of CE hours prescribed by licensing board • Volunteer at a Free Clinic a minimum FOUR hours per month; maximum THIRTY-TWO hours per month

  4. Program Rules Continued • Free Medical Clinic – Code of Alabama 1975 • Provide treatment, diagnosis and advice without a fee or compensation • Limited to care that does not include the use of general anesthesia or require an overnight stay in a health care facility

  5. Information • Application is available by contacting Carolyn Bern at carolyn.bern@adph.state.al.usor334.206.5436 • Coverage runs January – December 31st for one year • New application required after December 31st • Coverage is free to retired volunteer
