  1. Blues on Call A Service of Highmark, Inc.

  2. What is Blues on Call ? A trial program presented to PPG by Highmark (BCBS) to offer the enhanced services of Blues on Call to Active Employees and Retirees under age 65, along with Covered Spouses and Dependents, who have their Health Care Benefits through BCBS.

  3. What is Blues on Call ? Blues on Call uses all available data from Claims and Wellness Checkpoints (HRA’s) to identify people who: • Are living with Health Conditions • Are at-risk for Developing Health Conditions • May benefit from Lifestyle Improvement Programs

  4. What is Blues on Call ? Highmark Members are now asked for a consent to have their Wellness Checkpoint (HRA) data passed to Highmark…Why?? • Wellness Checkpoint (HRA) Data can provide information about opportunity for lifestyle improvement and risk reduction BEFORE health issues develop or worsen!!!

  5. What Services are Available to Members? • Outbound Calls are made to members who may benefit from the support of a Health Coach (Registered Nurse) • The members who receive outbound calls are identified via claims and Wellness Checkpoint (HRA) data analyses that are done on an ongoing basis • Health Coaches can offer members individually-tailored support, education, and access to resources that assist members in managing serious or chronic health conditions or assist members “at-risk” for developing conditions to improve their modifiable risk-factors • Thus far, 423 Outbound Calls have been made to 128 employees

What Services are Available to Members? • Health Coaches are also available 24 hours a day to take Inbound Calls from members. • Members can get support and education on any health related topic • Examples of Health Coach services include assisting members in framing questions before a doctor visit, answering questions that may arise after a visit, providing education around a new diagnosis or treatment, and offering access to resources including DVD’s, brochures, and on-line tools • Thus far, 4 Inbound Calls have been received by BCBS

What Services are Available to Members? • On-line Programs are available via the Highmark Website (www.highmarkbcbs.com) • Topics covered in the on-line programs include depression, weight management, nutrition, stress management, smoking cessation, insomnia, back pain, diabetes, and chronic conditions • The Highmark website also allows members to access a variety of information such as Discount Programs, Health Encyclopedias, Personal Health Record, and Provider Directories

What Services are Available to Members? • Wellness Coaching isalso available to members who are looking to make lifestyle changes • Wellness Coaches are Health and Wellness Professionals who offer members one-on-one support and tailored programs that help members make behavior changes that help reduce the risk of lifestyle related diseases and improve overall quality of life • Members who may benefit from Wellness Coaching are identified via their responses to the Health Risk Assessment. The Wellness Checkpoint (HRA) is a vital piece of the Blues on Call program, as it allows us to identify ‘at-risk’ members much earlier than claims generally allow. • Members who are interested in Wellness Coaching may also call directly to Blues on Call.

What Wellness Coaching Programs are Available ? • ‘Simply Well’ – Nutrition Program that focuses on a balanced approach to healthy eating • ‘Aim for Change’ – Weight Management Program that focuses on a balanced approach to nutrition and physical activity for successful long-term weight management • ‘Get Moving’ – Physical Activity Program that focuses on the benefits of exercise to health, fitness, and overall well-being • ‘Restoring Balance’ – Stress Management Program that focuses on introducing skills, strategies, and relaxation techniques to increase resilience to stress and improve ability to cope with everyday stressors • ‘Tobacco Cessation’ – Tobacco Cessation Program that focuses on support, behavior modification, techniques to manage withdrawal symptoms, stress management, and weight management concerns associated with quitting smoking. **Members who enroll in this program are eligible to obtain an 8 week supply of Nicotine Replacement Therapy at the low cost of $25!!
