  4. Meeting Overview Notification of change to the Request for Qualifications are posted on our website: https://www.phoenix.gov/streets/procurement/current-opportunities It is your responsibility as an RFQ holder to determine, prior to submittal, if notification letters have been issued

Vendor Registration All Firms MUST Be Registered in the Vendor Management System PRIOR TO SUBMITTING A PROPOSAL NEW FIRMS ---After Registering--- the City Will SEND AN EMAIL WITH A VENDOR NUMBER WITHIN TWO DAYS NEW THE VENDOR NUMBER MUST TO BE INCLUDED ON THE COVER OF THE STATEMENT OF QUALIFICATIONS OR ON THE BID PROPOSAL PACKAGE/ENVELOPE.   7. Project Description Seeking qualified consultants to provide Construction Administration and Inspection Services for: Rehabilitation of the City of Phoenix PVC-lined Concrete Sanitary Sewer Pipelines & Manholes.

  8. Project Description Rehabilitation of: • Approximately 41,200 feet of sewer pipelines • Approximately 188 manholes

  14. Statement of Qualifications Criteria • General information (15 points) • Design Experience of the Prime Firm • (8 points) • Construction Administration and Inspection Experience of the Prime Firm (17 points)

  15. Statement of Qualifications Criteria • Experience of Key Personnel and Subconsultants (20 points) • Project Understanding and Approach (30 points) • Overall evaluation of the firm and its perceived ability to provide the required services (10 points)

Statement of Qualifications Criteria • SOQ Cover Letter • Clearly display the firm name, vendor number, project title, and project number on the cover of the SOQ and submittal package. • Evaluation Criteria (including project organizational chart) • Additional Content (resumes or other information)

  17. Statement of Qualifications Criteria • MAXIMUM pages permitted is 12 pages: • + Evaluation Criteria (including organizational chart) • + Additional Content (resumes or other information) • + Photos, charts/graphs or other evaluation content • = 12 MAXIMUM pages permitted

Statement of Qualifications Criteria • Each page side with criteria information will be counted. • Pages that have photos, charts, and graphs will be counted toward maximum number of pages. • Cover letter, front and back covers, Table of Contents and tab dividers will NOT be counted toward the maximum page limit, UNLESS they include information that may be considered by the selection panel.

  20. Grounds for Disqualification • Receipt of SOQ after the specified cut-off date and time • Too few copies of the submittal • Deposit of SOQ in the wrong location • Violating the “Contact with City Employees” policy
