Important Information • All concussions should be seen by a licensed physician that is trained in concussion management prior to returning to play. • Tool for Concussion Management • Supervised Baseline Test • Post Injury Test If Needed • All Post Injury Tests should be taken at the direction of the physician.

  3. Age Requirements Individuals between the ages of 10 and 59 can take the test.

  4. What You Will Need • Quiet Space • Parental Supervision • Dimmed Lights • Computer(Preferably Desktop) with Internet Access. • External Mouse and Full Size Key Board

  5. How To Perform A Baseline Test • Type in the following address: www.impacttestonline.com/carolinasmedicalcenter • Follow the directions that are outlined on the following slides.
