Utah’s HP2020 Process • Expand on the IBIS-PH HP2010 process • Develop a relational data base that includes: • Divisions, Programs, and Partners • Statewide and sub-populations • Searchable, Multilevel, Interactive • Integrate HP2020 relational data base with IBIS-PH. • IBIS-PH Community Profiles • PHOM-like reports for LHDs • Prioritization of objectives

Grant Opportunity • State Action: Evaluating the Healthy People 2020 Disease Prevention and Health Promotion Agenda • Propose & conduct innovative and participatory strategic planning activities and processes with an emphasis on multi-sector collaboration. • Use HP2020 framework and concepts. • Web-based applications to support HP2020 Plans
