Focus on Person-Centered Planning • Process-oriented approach to empowering people with disability labels • Focuses on people and their needs • Individual is empowered to determine the direction for his/her life, not on the systems that may or may not be available • Ultimately leads to greater inclusion as valued members of both community and society

Vision LifePrep@Naz will: Provide opportunities for individuals with intellectual disabilities to continue in their education and development with peers on a college campus Influence the collaborative communities that come together by: Raising the level of expectation for individuals with intellectual disabilities Broadening the construct of diversity to include ability Preparing each participant to obtain meaningful employment

  8. Mission The mission of LifePrep@Naz is to provide community learning experiences that allow for the transformation of individuals with intellectual disabilities from students to self-determined contributors in our society

Year One Course of Study - sample • CCC 101 – Functional College Communication I (3 credits) • FSC 1XX – Functional Skills Elective (3 credit) • PED XXX – Health and wellness • FYS 101 – Freshman Seminar + Lab (3 credits) • VOC 101 – Vocational Experiences (3 credits) • CCC 102 – Functional College Communication II (3 credits) • FSC 1XX – Functional Skills Elective (3 credit) • PED XXX – Health and wellness • SER 101 – Service Learning (3 credits) • NAZ XXX – Nazareth Elective (3 credits) • VOC 102 – Vocational Experience (3 credits) Semester 1 (12-15 credits) Semester 2 (12 -15 credits)

  13. Non-resident participation • LifePrep@Naz is designed to facilitate the participation of neighboring school districts • 15:1 Special Class, including some adult ARC clients

  14. How do we prepare our students? Within a PCP framework: • Academic rigor • Independence • Ability to work with a variety of people • High levels of self-advocacy • Access to electives based on student selection
