Westminster Woods… Environmental Camp October 14-17, 2014 Paperwork All forms must be completed and returned to school by September 5, 2014 Total Amount $290 (tuition, transportation, food, housing) Medical Release Form (pink) Over the counter and/or prescription Inhalers in backpacks with doctor’s note (recommended) Medications will be collected 1 week prior to trip Parent Chaperones Needed! Rewarding and Demanding Experience Must commit to supervising entire week Tuition for Chaperones Supervise and ride bus
