  2. Description: A worker performing excavation with proper protection. Hazard: If anything was to happen, there is no means of egress. Non-Compliance: Subpart-M CFR1926.651(c)(2) Means of egress for trench Excavations. Sub Part P – Excavations - A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. Figure 1: Excavation & Protection Compliance: Subpart-E CFR1926.100(a) Head Protection. Subpart E – Personal Protective and Life Saving Equipment - Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.

Description: Worker is attaching stanchions on edge of precast hollow core concrete slabs and steel beam top flanges , stanchions to provide fall protection to construction workers on roof but fall protection for himself. Hazard: No fall protection Non Compliance: Subpart-M CFR1926.501(b)(5) “Formwork and reinforcing steel." Each employee on the face of formwork or reinforcing steel shall be protected from falling 6 feet (1.8 m) or more to lower levels by personal fall arrest systems, safety net systems, or positioning device systems. Each employee engaged in the erection of precast concrete members (including, but not limited to the erection of wall panels, columns, beams, and floor and roof "tees") and related operations such as grouting of precast concrete members, who is 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, or personal fall arrest systems, unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. conti…..

  4. Conti. Description: Worker is attaching stanchions on edge of precast hollow core concrete slabs and steel beam top flanges , with out fall protection. Hazard: Worker does not have fall protection. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502. Note: There is a presumption that it is feasible and will not create a greater hazard to implement at least one of the above-listed fall protection systems. Accordingly, the employer has the burden of establishing that it is appropriate to implement a fall protection plan which complies with 1926.502(k) for a particular workplace situation, in lieu of implementing any of those systems.

Description: Construction methods, construction safety, fall protection • Hazard: Worker does not have fall protection. • Non-Compliance: • Subpart- H • CFR1926.250(b)(5) Material shall not be stored on scaffolds or runways in excess of supplies needed for immediate of operations. • CFR1926.250(b)(6) Brick stacks shall not be more than 7 feet in height. When a loose brick stack reaches a height of 4 feet, • it shall be tapered back 2 inches in every foot of height above the 4-foot level. • CFR1926.250(b(7) When masonry blocks are stacked higher than 6 feet, the stack shall be tapered back one-half block per tie above the 6-foot level. Subpart-H CFR1926.250(3)Aisles and passageways shall be kept clear to provide for the free and safe movement of material handling equipment or employees. Such areas shall be kept in good repair. CFR1926.251(c)Housekeeping. Storage areas shall be kept free from accumulation of materials that constitute hazards from tripping, fire, explosion, or pest harborage. Vegetation control will be exercised when necessary.

  10. Description: A worker performing curb pavement of street, but with no traffic control or protection from on coming traffic. Hazard: Worst-case scenario, worker get hit by oncoming traffic. Non-Compliance: Subpart-G CFR1926.200(a) Signs and symbols required by this subpart shall be visible at all times when work is being performed, and shall be removed or covered promptly when the hazard no longer exist. CFR1926.200(c)Caution signs shall be used only to warn against potential hazards or to caution against unsafe practices. wording.
