  2. POSITION DATE OF COMMENCEMENT SALARY PACKAGE

  3. OTHER BENEFIT Employee is eligible to be paid F&B allowance when his/her is required outstation works. i) F&B will provide by Employer OR ii) RM30.00 per day if Employer not provides the F&B. Note : Outstation Area is excluding area phone code 03.

  4. PAYMENT OF SALARY Salaries are paid monthly directly into your bank account. A salary slip will be provided to you each month detailing payments for your references.

  5. PERIOD OF EMPLOYMENT PROBATION REPORTING

  6. HOURS OF WORK (DAILY) Management : Monday – Friday from 9.00am – 6.00pm Production : Monday to Saturday from 10.00am to 6.00pm

  7. HOURS OF WORK (AFTER EXTRA WORKING HOUR)

  8. The Employee may come late into the office after doing extra working hour However this must agree by superior before Employee can allow to coming late and this is subject to regular review of business needs (such as rehearsal time/meeting/discussion with client) so that this permission is denied.

  9. SHOW ALLOWANCE Extra working hour is + 8 hours not including break time taken.

  10. DUTIES AND RESPONSIBILITIES EPF SOCSO INCOME TAX

  11. ANNUAL LEAVE An employee requesting for annual leave has to submit the Application Leave Form to Management at least seven (7) days.

  13. The Company shall bear your medical consultation and medication for not more than a maximum of RM25.00 per visit for all clinics except Company approved clinics/panel doctors and government hospitals where there is no limitation. The medical benefits do not include dental, obstetrics, general medicine test, cosmetic surgery and ophthalmic treatment.

  14. PUBLIC HOLIDAY Employee shall be entitled to paid holidays for all gazetted Federal and State public holidays applicable to the Federal Territory (Employment Act 1955 & Holiday Act 1951).

  15. OTHER LEAVE All leave excluding rest day and public holiday except Maternity Leave. Bereavement Leave : Employee must produce a copy of death certification

  16. ABSENCE FROM WORK If for any reason you are unable to report for work, or if you will be late, you must notify your immediate superior at least two hours before your scheduled shift so that arrangements can be made to take over your work. Do not just leave a message with a co-worker. Failure to notify your immediate superior of your intended absence could be cause for disciplinary action to be taken against you. Emergency Leave shall attach together with authority proof.

  17. NOTICE OF TERMINATION This contract of employment shall be deemed to have broken if you have been continuously absent from work for more than two (2) consecutive working days without prior leave approval from the Superior

  18. CONFLICT OF INTEREST You are expected to carry out your duties and responsibilities diligently and shall, at all times safeguard the interest of the Company. The Company regards violation of “Conflict of Interest” as severe offence, which may lead to dismissal

  19. CONFLICT OF INTEREST You are required to devote yourself exclusively to the services of the Company and shall not be engaged in any other business or occupation without prior written consent of the Company.

  20. CONFLICT OF INTEREST You shall not make any personal profit in the course of your employment with the Company and shall not either directly or indirectly receive from any person(s) supplying goods to or dealing with Company any gift, bonus, commission or receive gratuities on personal basis from well-meaning guest or patrons in appreciation or recognition of good or exemplary services rendered by you.

  21. CONFIDENTIALITY The confidentiality of information is important. You must take every precaution to protect the image of the Company. You shall not during, or even leaving the Company, divulge or make use confidential information for personal gain. You are also agreed not to discuss your salary with any team member.
