GPs in Community Health Services Strategy Project Objective: • To improve GP access and integration with allied health and community health nursing by undertaking a systematic and coordinated approach to care planning and care outcomes • Residents of pension-level Supported Residential Services (SRS)

  3. GPs in Community Health Services Strategy Issues: • Multiple chronic & complex conditions • Vulnerable target group • Multiple GPs / agencies 4. Lack of communication & coordinated care planning  Service duplication/falling through the cracks 5. Reactive health care provision

  4. GPs in Community Health Services Strategy Setting the Scene: • 6 pension-level SRS with a total of 103 residents • CCHS - Adult and Outreach Team • 27 GPs from 18 practices • Multiple health & recreational services

  5. GPs in Community Health Services Strategy Strengths: • Specific, well-defined target group • Commitment to improving health and wellbeing of SRS residents • Many GPs providing a service • Many services being offered • SAVVI

  6. GPs in Community Health Services Strategy Current Care Planning: • SRS care plans (r/v 6/12) • Discipline-specific care plans • Some intra-agency care planning • Occasional issue-specific inter-agency care planning  Ad hoc/informal as issues arise

  7. GPs in Community Health Services Strategy Where are we at now with ICP? • Planning phase  consultation, forum and research • One working party meeting • membership = SRS GP, CCHS, ISCHS, Waiora CMHS, RDNS HPP, SAVVI, and Southern Citizen Advocacy

  8. GPs in Community Health Services Strategy Relationship and Capacity Building: • GPs/PNs - GP Advisor & GSEDGP PN - GPMP/TCA up-skilling - practice visits by CCHS • SRS proprietors/CCHS - joint in-service • Other agencies - consultation - project partners

  9. GPs in Community Health Services Strategy Challenges: • Getting people together • Engaging GPs effectively • Staffing changes • Lack of human resources/key worker • Lack of IT infrastructure

  10. GPs in Community Health Services Strategy The Vision: • Develop an SRS ICP and documentation model  trial at one SRS Step 1: CCHS, GP, SRS Step 2: other GPs/agencies/residents • Evaluate and modify • Implement model with 5 other local pension-level SRS
